I'm proud wife of DarkEnergy, my husband darkEnergy Diagnosed stage4 metastasis with PSA 1000+ was August 2018, after 3 months of Zytiga, Lupron plus he added chemo to it he's psa became undetectable to 0.02 , two years later my husband decided to stopped ADT to see what kind of cancer cells he has, so he asked he's doctor if possible to stopped he's medication and the doctor said okay let see what happened π€ but the doctor said we need to monitor your psa every 3 months he added. Yesterday he got the results of he's psa test it was 0.6 and Testosterone is 9 so what's this means? Is this called castrate resistant?π’ππππ

One's PSA has to go above 2 before one can be considered castrate resistant.
Here's the official definition of castrate resistance from the American Urological Association:
American Urological Association (AUA) defines disease progression as β₯ 1 of following progressionsbiochemical progression, defined as presence of all of following
continuous increase in PSA, measured at β₯ 1 week apart
PSA > 2 ng/mL
radiographic progression of new or pre-existing disease
clinical, symptomatic progression

Casey ...Read carefully this. Here is a clear answer. When Lupron and Zytiga is taken..this combination shuts down all 3 sources of Testosterone (androgen) production. And therefore, his PSA went down to 0.02. The reason ? In absence of Testosterone, both the normal prostate cells and Prostate Cancer cells shut down (go into total lock down) Then, after he stopped Lupron and Zytiga, his remaining normal prostate cells started opening up and slowly began the work of PSA production...(like partial lock down with some essential services opening up)
Means at present his PSA of 0.6 is most likely coming from his normal prostate gland cells as his prostate is still there fully intact.
So when we will know, Up to what level of PSA is likely coming from normal prostate cells ? It depends on size of his prostate . Size of prostate is determined by an Ultrasound of prostate.
Lets assume, his prostate size is 30 ml. In that case, PSA up to 3.0 can be considered as coming from Normal prostate cells. For this calculation to be correct, His prostate has to be intact, his general health has to be very good and his Alkaline Phosphatase level has to be in normal limits. Tell him to enjoy his vacation and not to worry until PSA goes above 3.0. Should monitor his PSA and ALP every month and not every 3 months. Its my opinion . not a medical advice.
I think that, in this particular case, the fact that the PSA has gone up is almost certainly due to stopping the drugs, not to the drugs failing to work. Your husband's PSA isn't going up because he's castrate resistant, but because he's no longer castrate. I suggest that you consult with the oncologist, but I expect him to recommend getting back on the drugs.
I don't know what advice to give you. It may be that you and your husband hate the side effects of the drugs so much that you are willing to risk a shorter lifespan to get a higher quality of life. And it is conceivable that Dark Energy will live just as long with intermittent therapy as with continuous treatment. But getting back on the drugs until they stop working seems to me to be a safer course from the point of view of longevity.
It's a terrible dilemma.

What a lovely couple and what a beautiful home. Where is that Seriously I am going through the same quandry. On ADT but PSA is dropping very slowly. My Oncologist says that as long as it is not going up we are not going to worry for now.
Castrate resistant metastatic prostate cancer is when hormone deprivation therapy, and other interventions stop working. Also, it has been confirmed by new metastases by Pet scan. PSA must be confirmed by several higher readings. Only the Medical Oncologist can answer your question.
No it means "castrate sensitive". It means that as the EDT meds wear off, the testosterone levels rise to normal, and more T means faster prostate cancer cell growth because they are sensitive to T. Castrate "resistant" means PSA rises even when T remains low.π Yes, nice house, nice pic. Best wishes. I was on four year vacation and PSA rose to 35. Return to therapy put me back to <.1.
