Hello fellow warriors. Metastatic, Gleason 9, many mets. On Zytiga for about 2 years with undetectable PSA. Two months ago a rise to 0.4, and then 6 weeks later it doubled to 0.8.
I assume such a velocity means Zytiga is failing. Is that right? Thanks!!!

It's still low. Unless you have pain or other symptoms, stick with it. If it reaches 2.0, get a new bone scan. Try switching from prednisone to dexamethasone.
Thanks, TA. You are a gift to all of us! I appreciate your straight answers.I'm frustrated by the false positivity being pushed on me by family and friends. I get that it helps some, but I prefer to know what is coming so I can plan accordingly. Not giving up - just a realist.
Curious what others think: it seems that on average all the various drugs like Zytiga, etc., really only add a matter of a few months in cases like mine:
Stage 4 metastatic (too many mets to count, including as far away as skull and shoulders), nearly 80 years old, gleason 9. Treatments have left me incontinent (a little better after TURP and some other treatments), impotent, severely diminished muscle tone despite weight lifting and serious exercise, and a big belly. I figure next after Zytiga is chemo, and wondering if worth it for a few extra months. I know there are outliers, but everything so far points to me being middle of the pack for my diagnosis.
If you are not in too much pain and can still appreciate the beauties of life and the presence of family and friends, then say yes to some chemo when indicated. It is not so bad and may restore responsiveness to abiraterone. Precious days to you.
Chemo, Zytiga, etc. add years to life if used early enough, but only months if used for end-stage disease. The tragedy is when patients don't use them early enough.
Plus, they relieve pain and enhance quality of life compared to patients who don't use them. You may imagine you might feel better without them, but sadly, that's not what ever actually happens.
Hio jersy. I'm 77 and will have to make decisions when psa is doubling. I've been on Zytiga 3 yrs. I'm prepared to go with Chemo but did your mo discuss Provenge?
