I have a teleconferencence with Dr beers at OHSU on Tuesday. My oncologist at knight cancer is very concerned, because historically when my PSA has risen Mets have appeared in 5 areas two small ones on my cranium and humerus and a met on my rib all occured quickly with a lesser value PSA around 3.0 to 6 . This is very discouraging and concerning to say the least, my 4 year anniversary of my dx. Has just approached I started with a rad prostectomy then 37 sessions of rad, Then zytiga 15 mos of bliss no rise in psa, then all good things end a jump up, then xtandi lasted two months. Then a L3 met zapped off Sept 2019 yay . But then a rapid 4 more came back on board. Now chemo with two consecutive rises t from 2.7 to 9.3 . It was suggested xofigo, and if anybody knows of something different or a combination of above mentioned drug. I know I have some options left, although things seem to be accelerating pretty fast. I realize everybody's journeys different but perhaps I can rally some warriors for insight or personal experiences, it would be very much appreciated.

4th chemo session of docetaxal PSA increased from 2.7 to 4.4 to 9.3 in three consecutive sessions every 3rd week.
Xofigo is radium 223 which is injected into your arm. I am under going treatment now. No side effects except a little diarrhea after each dose. I was terrified of xofigo but it's all good.

You can switch to Cabazitaxel if Docetaxel is not effective. Have you had a biopsy to look for treatable mutations and neuroendocrine PC development?
There is also LU-177 if you are PSMA avid. There are losts of post here on that if you are not familiar.

Yes mutation showed bracha 2 and some possible treatment options ,that was in September and that want to do another one in humerous, although the 1st was extremely painful, I said I dont mind going on 2nd test if valuable info was gathered on the 1st one, no one could definitively answer that yet, I will ask prostate specialists Tuesday. If you have a BRCA2, you could be treated with a PARP inhibitor such as Olaparib or Rucaparib.
I too am in my fourth years from Dx, had an RP, docetaxel with Lupron, was BCR in seven months. Had Axumin Scan that showed tumor near bladder and one bone met on my left femur. I had SBRT for bone met and I have been taking Zytiga for about 18 months with PSA at .05. I don't know what your appointment will yield but docetaxel wasn't effective for me. The Lupron worked for a short time.
I recommend you just take a deep breath and continue investigating what options are out there and bring them to your appointment to discuss. Dr Beer my have some good options. Anticipating is the worst, so talk to family and friends this weekend and get outside if you can do so safely. Try to be in this moment, now, and enjoy it as much as you can.
