Moving on up,whats next.

Diagnosed 2016 stage 4. 8 gleason 12 psa. Rad prostectomy 37 sessions radiation lupron casodex etc. The past 13 mos on zytiga and elegard doing well tolerating for the most part. Although i went from 4 mos ago from a 0.006 feb 2019 undetectable psa to a 0.009. Still no concern to now 5/24/19 to a 0.065 i realize its a small number but is zytiga becoming less effective? And my doc says chemo is a long ways off but whats next if this uptick continues. And what are some scientific proven things i can do to slow the progression. I have read many of your post and know it could be worse although its relative to my experience of undetectable serenity and perhaps some denial. Thankylu in advance gor your input.

Doing the Ultrasensitive is bound to create anxiety....who knows?? This last test in May could be lab error...I would do a retest and consider adding indomethacin and/or switching from prednisone to dexamethasone, as shown in these studies:

I would stay on the abiraterone for as long as possible and try these strategies to prolong effectiveness...The number of drugs is slowly increasing, but maximizing their potential is essential in this war...

After SRT, it makes sense to switch to a conventional PSA test. There is nothing you would do differently at these low numbers, so they cause anxiety needlessly.

Chemo may or may not be a long ways off.. When Zytiga definitely fails, Xtandi might work for a while at least...Are you still being treated by a Urologist? if so, it might be time to bring a Medical Oncologist into the picture. They know a lot more about this stuff..

Thankyou i have been with a oncologist for about a year now. She stated chemo is far down the road when mets have dug in to the liver etc. I just seen a third in a row uptick in PSA and got concerned to put plan b into action soon if the trend continues

Just my info to add, I live by 3 digit psa. Diagnosis at 56 PSA 2.4 very aggressive! RP got me through 1year4 months with psa at .1 jumped to .4, 14 days .5 10 days .6 42 radiation treatments,ADT (trelstar) 2 years 1 month. For18 months .006 next psa .030 next psa .235 I'm right back where I was at 2.1 years ago ca growing fast. (Aximun)Pet scan next week MO meeting next week (Zytiga) was mentioned? I really felt uPSA provided me with an relaxed feeling knowing if it was moving I would know it sooner and have a plan in place! Maybe just me? I'm 60 now.

You understand that doubling of your numbers seems to be what they really look for.. PSA can tic up and down...I really think you should listen to your OC on this one...Stressing out will be harder on you than just taking the glass half full strategy...If you get down just reach out to J-O-H-N....He is our resident comedian...He is not much but he is all we got!!! Not much but he is all we got!!! The court jester. A cut up. I'll have you know that j-o-h-n is actually John, The King of Comedy. Here in disguise, like a Shakespearean Tragedy. You must show him respect, and at least giggle at his jokes. I believe he is taking names, and has a list. There will be an end time. And The King will be cutting off heads and laughing hysterically.

I like NP Fish answer the best. I am in similar position. The switch to dexamethasone, adding Indomethicin are good options and I will try if PSA hits 0.1 twice in a row .I have PSA to 3 decimals... I like it and I don’t stress I just watch the trends.

Serenity is _important_ so don't feel shy about admitting its value and the importance of attitude in maintaing your well-being. I am ambivalent about often I should check my PSA for just that reason. I wish you well.
