Husband has extensive bone mets and has had chemo and zytiga. 3 years since diagnosed . Now scans show renal neoplasm on left kidney. What is the standard of care on this? We realize he has a very bad cancer and don’t want to put him thru extraordinary measures . He wants to enjoy life now.

Psa gradually going up has been on zytiga 18 months. 8.5 psa
Has he had Jevtana? He can get by with one functioning kidney.
No jetvana. Is that is hard on you as docetaxel ? He doesn’t want any more chemo . He said he lost six months of his life and it doesn’t seem like it extends life very much longer. At most he will most likely get 2 years but he wants to remain active and then go quickly.
I’m sorry that he is suffering so . Tuff decisions , None really clear cut . We all want to remain active then go quickly . QOL is important. This treatment game we play is roulette .
He might try Provenge.
We did meet with a geneticist at hospital and they did blood tests. Found no mutations. They have now mentioned a bone biopsy to find mutations.
If his cancer is PSMA positive (it could be determined by biopsy, or PSMA PET/CT, Ga 68 or DCFPyl), he could consider treatment wit Lu 177 PSMA and or Ac 225 which could help to control the cancer for a while with much less negative side effect than chemo. He could, if possible financially go to Germany or Australia or consider some of the clinical trials in the USA,

Biopsy and genetic studies could be also useful to determine if the cancer could be treated with specific drugs, such as Keytruda, Olaparib, Rucaparib etc.
