WHEN IT RAINS,IT POURS

Hey ,my hubby58,has stage 4 prostate cancer.he was diagnosed in July 2017.he has had the casodex,and xtandi,radiation and now today we were told that he has lesions on his bones,a spot on his liver,spa is over 41..he also has mestatic cancer lymph nodes swollen..please tell me what can I do to help my husband..

Docetaxel might be a good next step.

I am not sure what Xtandi is.I have seen it still it doesnt resonate with me. He should be taking 150mg of Casodex daily and also LUPRON shots Monthly to drive down his PSA.

Finasteride or Avodart helps too if he is hormone sensitive. Oral bisphopanates can drive lesions from the bones...Zometa is one thats an IV.Use with caution. It can cause jawbone issues if dental work is done.

Ok,the dr took him off the casodex, cause it stop working. That’s when was put on the xtandi and now it’s stop working too..now the dr is putting him on zytiga and prediszone in a few days and he has to have a shot once a month for his bones.. we live in a Augusta Ga..and our oncologist retired and turned us over to someone new..and I’m really frustrated..

Sounds like time for docetaxel or cabazitaxel. This chemo is generally tolerated quite well.

prednisone is a steroid and has to be used with discretion. What dosage was the Casodex taken in? I was on 150mg daily whereas most get only 50mg and thats not enough. Admittedly I wasnt in your situation and it was a long time ago.

It was a time when chemo was starting to be used. I missed all of that and although at the I thought I was getting serious meds I was wrong. But my PSA dropped like a rock and it stayed very low, very long. Take a look at the Compassiate Oncology site and see what you can find out about your situation.

Ok I will,thanks,he was on 150 mg casodex.it work for a while,the psa starts to go back up again

Suggest you might ask your MO about a AR-V7 test before beginning. If he has failed Xtandi, excellent chance will immediately fail Zytiga. A negative reading on this test will suggest may have a chance that Zytiga will work,,,a positive reading strongly suggests that will be valueless.

No reason to expend money and incur SE’s of Zytiga if test shows positive.

You may have to educate your physician on this,,,many do not know.

Just google AR-V7 for more info.

Have heard Zytiga after Xtandi usually doesn't work for long. Just what I heard. Doctor has not mentioned chemo? Don't be afraid of chemo. Almost everyone on this site has had chemo. Good luck going forward.

Ask here if anyone knows of a good MO in Augusta or nearby. I've seen many doctor's names mentioned in Atlanta. (Spent some time in Ft. Gordon - now Camp Gordon).
