2020 The Arbiterone I started in July has stopped working. My last set of scans in February showed tumors stable with no new Mets. PSA back up to 15.7. My medical oncologist has recommended in-home hospice. I enrolled this past week. I am now on Palliative care only. A summary of my treatment since diagnosis in 1999 is in my profile. I hope it is helpful to others recently diagnosed as to what the future may hold.

What an *amazing* fight you put up. You didnt cede an inch, not one damn inch! There were no complaints and all the info you shared. There is never a good thing to say when you read these statuses, but I am in awe sir.

It seems you never had chemotherapy.

You could discuss doing chemo with docetaxel or cabazitaxel and direct biopsy of the metastases to study the genome of the cancer and try to determine if there is neuro endocrine PC.

If direct biopsy is not possible they could do a liquid biopsy to study circulating tumor cells or free DNA particles. With a PSA of 15 and multiple bone metastases the liquid biopsy could have a good yield.

If they can study the genome, the cancer could have mutations such as BRCA, ATM, MSI-H, MMRd etc which could be treated with drugs such as Olaparib or Keytruda or similar drugs. If there is a neuro endocrine component they could try cabazitaxel plus cisplatin chemotherapy.

Then there is the possibility of getting treatment with LU 177 PSMA and Ac225 in Germany if financially possible.

I agree. Are you healthy enough to take a run at chemo? That would be the next logical step where I in your position.

You've been fighting those tiny M.F. bastards for 20 years. I wish I could recommend something so that you can be fighting them for another 20 years. Just laugh if you can and I hope and pray you stick it out and win the battle. God Bless...
