Hi fellow warriors! I was dx in May 2021 with pca. cT3a cNO cMx. Psa 296 GS 8.My Urologist placed me on Bica. for 20 days. Did a Ga 68psma pet/ct scan where it showed an enlarged prostrate and some reflections on 3spots on my spine and one on my left acetabulum which they couldn't concretely say it's mets but only assumed with high probability that they were. At the end of Bicalutin I started with a 3monthly Lupron ,( I have had one implant till now) . A month later I started docetaxel chemo + 5mg prednisone (1×daily)with psa 22. After the first infusion the psa dropped to 0.8 , had second chemo on 5th of August. My question is; is it normal for the psa drop that fast and if so or not, what does that mean in terms of prognosis ?

I'm not sure what accounts for your drop in terms of the amount of time and the specific treatments for each time period between the two PSA tests. It is clear however that you are responding very well to the treatments. That is a very good thing. As I understand it, in general, patients who reach lower than average PSA values in less than usual times will get longer remissions from the treatments In other words, your PSA and your cancer burden will be lower, for a longer period of time, than men who have higher and slower responses.

Are they going to radiate the suspected mets? And the prostate?
No decision on that yet. I have to complete my 6 cycles of chemo before further planning.

You are doing all the right things to treat it at this point and with a good attitude towards life. That makes your prognosis excellent. The rapid and deep response is great. Consider and discuss adding a bone protective regimen early rather than later. Prolia or Zometa.
