Add Zytiga?

Diagnosed September 2017, age 68, PSA 147. Placed on Luprilide, Bicalutamide 50 MG daily, spot radiation and Xgeva. PSA went down in 2 months under 0.1, still u/0.1. The more I read the more I believe should press to add Zytiga. Any information, thoughts if I would still get a benefit taking now, given the delay, rather then waiting for PSA to rise?

CAT scans clear, extensive bone mets.

Thanks, Should have mentioned MO doesn’t want me on chemo do to other complications.

I read that Zytiga used a full year after the diagnosis still conveyed a very good benefit. Discuss Zytiga use with your medical doctor..

Hi Steve I've been on Zytiga as a first line of treatments along with Eligard, and Xgeva for bone Mets. I have been 0.1 PSA since one month after starting the medication its now six months into it. Zytiga isn't a wonder drug but has worked wonders for me. Best of luck. Never give up never surrender. Leo

What does your mo say? You have time to go on zitiga, I have been on it for 17 months,my psa is going up now, it only works for so long, I would take when needed, only my thinking, again ask your mo that is why they get the big bucks

Conflicting guidance, MO normally puts everyone on Zytiga, however says many patients don’t get below 0.1, he’s a bit conflicted. Have had consult with mark Moyad in past, sent him latest testing for feedback. Given his schedule he usually takes a while to get back.

I would recommend you pursue Zytiga. I have been on it less than a month so far, and PSA has dropped 90%. I will start Xgeva in a couple of weeks at my next MO appt to further control bone mets. Make sure to check your insurance for Zytiga as it is very costly, ranging from $4 to $9 thousand per month. I understand a generic is in the works, but not for a while.

Spot radiation to extensive mets?

I am oligometastatic and have been Lupron and Bicalutimide (PSA<0.02, T=6) for nine months. I just returned from an appointment at JH and the MO recommends adding Zytiga for six months then stopping to watch the numbers. My numbers are good, but he agrees to "intensify" treatment because I am pushing to be aggressive. In my case, it is extrapolating from the trial results because those were mostly based on multiple mets and castrate resistant men. But I think it makes sense.

In your case, I think it makes even more sense to add Zytiga and kick some ass! Good luck.

About all I can add to this discussion is that I too have bone and lymph system mets, along with mets in the area my prostate used to be (six years since radical prostatectomy). I am about to complete 6 rounds of chemo (Taxotere) and at the end of September I will go right into Zytiga thanks to the LATTITUDE study results.
