Husband started on Xtandi 2/3/18. He had orchiectomy but hid testosterone isn’t as low as the urologist would like so he suggested to add Casodex.
I did a quick search and found no contraindications but wanted to check with the experts here.

Neither Xtandi or Casodex lower Testosterone...the URO meant PSA
Sometimes the terms get confusing and explanations are missing part of what you are looking for.
From the folks who make Zytiga: "The growth of prostate cancer is stimulated by male hormones (androgens / testosterone)...Zytiga®, blocks androgen production at three sources; the testes, the adrenal glands, as well as from the tumor itself."
Zytiga might also be an option for your husband.
Seems odd to combine Xtandi & Casodex - they are both androgen receptor [AR] antagonists. As such, they would be in competition for AR binding. As others have said, neither lower T, but that may not be the point of the doctor's suggestion. Perhaps, in view of his T level, the doctor is hoping for a more complete AR inhibition. There are no trials of course.
Avodart would greatly reduce the probability of T converting to DHT. Dr Myers has said that although castration therapy targets T, the purpose is to inhibit DHT, which is the real problem.

I have never heard of taking xtandi and casodex at the same time. And yes either should lower PSA readings. My recommendation would be to start with casodex and if that fails go to zytiga or xtandi.
He was on Casodex last year and taken off before starting Xtandi in Feb
Answering my own question, I just spoke to the urologist's nurse regarding taking Xtandi with Casodex. She said just yesterday the Xtandi drug rep bought the office lunch and the nurse was in on the discussion regarding taking both Xtandi and Casodex. The rep said it was okay to take them both but the nurse will verify and call if there is a contraindication.
Warning about Bicalutamide (Casodex) and Nilutamide (Anandron)

Here might me an opportunity. Here is my story and warning advice on Nilutamide and Bicalutamide:
My Oncologist switched me from Bicalutamide to Nilutamide and after about two and half month of taking it, Nilutamide side effect caused me Pneumonitis.
It was diagnosed, unfortunately, as bacterial ,non specific Pneumonia
and I almost dyed. I was loaded with many antibiotics,was on Oxygen
for 2 months,was given Corticosteroids and Nilutamide was withdrawn. Than finally I was getting better after loosing 20 Kg (about 44 pounds ) in 2 months.
I am Chemist,
so later I was researching what was going on. One of Nilutamide rare side effects, in about 2-4 % of patients may cause "Interstitial Pneumonitis",
pneumonia like condition. It is described in the medical literature,but you have to specifically look for it.
None of my attending physicians was aware of it !

Both Bicalutamide and Nilutamide affect liver functions, even Bicalutamide was reported that it may rarely cause Pneumonitis :

I am no longer taking Nilutamide or Bicalutamide ,have appointments with specialists to see what will be the alternative.
