Interpreting Blood Tests

Good morning.

I am posting my latest set of blood results in a table that also shows the history of blood tests that I have in relation to my hypothyroidism. I am slightly concerned that although I asked for a full blood test, the GP didn't test for Folate Serum, Free T3, Thyroglobulin Antibodies, or Thyroid Peroxidase Antibodies.

Should the missing tests be requested by me as a matter of routine?

Is anyone able to comment on my latest results and offer any advice?

I have chronic insomnia, am gaining weight in spite of exercising regularly and eating sensibly. I feel like I am in a rut and getting no support from my GP. I asked for a referral to an endocrinologist, but was told that my bloods are normal and there is no need for a referral.

Any advice would be appreciated.

Which brand of levothyroxine are you taking On dairy

Do you always get same brand at each prescription

Was this test done as recommended, early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

Free T4 (fT4) 17.4 pmol/L (11.5 - 22.7) 

Ft4 only 52.7% through range

Request next dose increase in Levo to 150mcg daily

Approx how much do you weigh in kilo

Guidelines on eventual daily dose is approximately 1.6mcg Levo per kilo per day

Antibodies are only ever tested once on NHS

Exactly what vitamin supplements are you taking

Serum B12 is far too low if under 500

Answers to the questions you pose:

Which brand of levothyroxine are you taking: Almus

Do you always get same brand at each prescription: Yes

Was this test done as recommended, early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test: Yes, complied with all conditions and test was taken at 0840.

Free T4 (fT4) 17.4 pmol/L (11.5 - 22.7) 

Ft4 only 52.7% through range

Request next dose increase in Levo to 150mcg daily

Approx how much do you weigh in kilo: 91 kg

Guidelines on eventual daily dose is approximately 1.6mcg Levo per kilo per day: Which would indicate a dose of 145mcg (in line with your recommendation to ask for an increase to 150mcg per day)

Antibodies are only ever tested once on NHS: Noted

Exactly what vitamin supplements are you taking: High strength Vitamin D

Serum B12 is far too low if under 500: Didn't realise that was low. I will look at your recommendations for a B12 daily supplement.

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement

A week later add a separate vitamin B Complex 

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

If Vegetarian or vegan likely to need ongoing separate B12 few times a week

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)

This can help keep all B vitamins in balance and will also help improve B12 levels too

B vitamins best taken after breakfast

Igennus B complex popular option. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose and may need 2 per day and/or may need separate methyl folate couple times a week

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) Thorne can be difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 5-6 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12 if last test result serum B12 was below 500 or active B12 (private test) under 70
