Hi I posted before with the blood results below : I have had the increase in Levo of 125 and 150 mcg since early October and after the last results they asked me to have more bloods done on 13/1/25 which was 6 weeks later .
done 26/11/25
TSH 3.80. Range 0.30-4.50
FT4 14.9 “ 11.0-22.0
FT3. 3.5. “. 3.1. - 6.8
previous ones on 15/3/23
TSH. 1.07.
FT4. 16.4
FT3. 3.5
these are my latest bloods done in 13/1/25
TSH. 0.29. O.30- 4.50
ft3. 4.1. 3.1- 6.8
ft4 17.7. 11.0-22.0
just wondered if anyone can give me any advise on if I am optimal and if I am how do I keep my TSH right I don’t know if I want to lower my dose ? Also now does it look like I’m converting t4 to t3 ?
Sorry for a jumbled message but it seems it’s going to be weeks til my Endo can look at my bloods and advise any changes etc

Optimal is not a number, it's how you feel. But I very much doubt you feel optimal with that low FT3. You're obviously converting some T4 to T3, but not enough. Your conversion is poor.
FT3: 4.1 pmol/l (Range 3.1 - 6.8) 27.03%
FT4: 17.7 pmol/l (Range 11 - 22) 60.91
As for keeping your TSH 'right', it's not about the TSH. The TSH is pretty much irrelevant. It's the FT3 the most important number. And why would you want to reduce your dose? If anything, you need to increase it. That might improve your FT3 a little, but probably not enough to make you well. What you really need is some T3 added to your levo.

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test
This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)
is this how you did your test
When were vitamin D, folate, B12 and ferritin results
Looking at previous post your endo was going to test these
What vitamin supplements are you taking
If not on gluten free diet
Get coeliac blood test via GP or endocrinologist BEFORE trialing gluten free for 3-6 months minimum

Serum folate. 4.9 2.5
serum total protein 75 60-80
Vit b12. 605 197 - 770

I didn’t get to see her after my last bloods I just got a letter weeks later to say repeat 6 weeks which was 10 days ago and I had to ring secretaryy to alert her to my results we only gave one Endo looking after the whole of Cumbria .
Igennus B complex popular option. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose and may need separate methyl folate couple times a week
Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) Thorne can be difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay
IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 5-7 days before ALL BLOOD TESTS , as biotin can falsely affect test results
In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg)
From memory you had your T3 taken away, didn't you, and the endo said she would reinstate T3 if needed ?
So, have you seen this endo again as it certainly seems that T4 only is not going to get your T3 up high enough, for you to know that you are optimally medicated and well !
Once on any form of thyroid hormone replacement the TSH is the least important of all the blood test results - and many of us when optimally medicated will have a low/suppressed TSH.
What is more important is that your T3 is too low in the range for you - especially if you have symptoms of hypothyroidism and it reads as though you do -
We generally feel best with the T4 up in the top quadrant of its range at around 80% with the T3 tracking just behind at around 70% through its range.
Currently your T4 is coming in at 61% with your T3 at just 27% through the ranges :
Do you have any current readings for ferritin, folate, B12 and vitamin D -
as no thyroid hormone replacement works well if these corner stones of good health are up and maintained at optimal levels.

Everywhere I researched suggested ferritin needed to be over 70 for any thyroid hormone replacement to work well -
I now aim for my ferritin to be up at around 100 :
Your folate is too low and needs to be up at around 20 -
Active B12 125 ( serum B12 500++ ) so presuming your result is serum and good - if you take too much you just pee it out - though you're not even supplementing - !!
and vitamin D needs to be up at around 125.

Yes, it's a very good result - and if you had been supplementing I'd have suggested that you saved some pennies by dropping down by a dose or two whatever B12 supplement you were taking !!
