I have hypothyroidism. I currently take 150 levothyroxine and feel good. I have had a recent blood test with TSH 0.30 miu/l and T4 23.5 pmol/l. I have received a message from GP to book an appointment to discuss .

What time of day was this test taken?

When was your last dose of Levo before this test?

Please add reference ranges for each test.

What supplements do you take and what were last vitamin test results?

Appt was 1.00 Pm. Reference range T4 11.5 - 22.7. TSH 0.49-5.23. Took levothyroxine in the morning. I take vitamin c , vitamin d with calcium. Never had a vitamin test. Thank you.

Refuse to reduce dose without further testing

Ft4 falsely high as you took Levo before test

TSH is lowest in middle of day

ALWAYS test thyroid early morning

GP often unaware of significance of timing of test

There’s no Ft3 test result

ESSENTIAL to test vitamin D, folate, ferritin and B12 at least annually

Request new test including Ft3 and vitamin levels

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

If you normally take levothyroxine at bedtime/in night ...adjust timings as follows prior to blood test

If testing Monday morning, delay Saturday evening dose levothyroxine until Sunday morning. Delay Sunday evening dose levothyroxine until after blood test on Monday morning. Take Monday evening dose levothyroxine as per normal

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 5-7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12

Teddym if 150mcg T4 feels right for you don't let the Dr lower your dose. There are no cellular tests. And labs are just a snap shot of the moment the labs where done. And it's not the only decision maker for your well-being. Best Wishes.
