I had Rai 8 years ago . Felt nit right on Levi. Tge usual story .so at great expense and with gp approval I did a deep dive into ndt etc and started taking it working it up slowly till I felt right depending o. Things about 1?or two grains . I have recently bull ill with gut issues ( long standing) but worse there were signs my liver might not be right had scans nothing to see but I do have non alcohol fatty liver . Anyway after not seeing a doc for six years thus all forced be back to gp and there we found I was way under active . Bring unwell I had taken my eye of the ball and also had been forgetting at times my second grain each day . Now I have to get my bloods ax right as I can and eluidcusyskkybdivthus till I felt right then privately get tested . I did the tests and hope to let you see them the tests . I gave give back to two grains but go won’t be happy as she is testing before the six weeks mark . One thing I noticed is that my vitb22 is high and that is off as I had not being taking it regularly at all . My cortisol is high mid day and at 2pm or was the day I did the saliva test . I use blue horizon . I should developed very very high blood pressure . I am also wondering if a very dry mouth might be attributed to being so low . Are there any papers I couid show my gp to help her understand why t4 alone cannot suit all sorry for such a long post . Am wzuru g on eye surgery , there may typos cheers lee

Hello Ravensgeo :

I can't see these results clearly- I too had RAI thyroid ablation back in 2005 and became much more ill some 8 years later with symptoms including a very dry mouth - and this is a known side effect of RAI as this toxic substance is known to cause such symptoms as blocked salivary glands.

I was treated with T4 monotherapy - refused both T3 and NDT in 2018 and then started self medicating with NDT and am much improved and look after myself outside of the NHS system.

It reads as though you felt too ill to continue taking your 2 grains of NDT and I am very sorry to read of what you are going through - but if you were well on 2 grains split - AM and PM - obviously this would be the dose to try and work back up to in the first instance.

On NDT your blood test results will look different to T4 only blood tests -

Is this the same doctor who monitored you when well on the NDT - so has the previous results and where your T3 needs to be for you to feel well ?

On NDT your TSH will likely be low suppressed at 0.01 - and your T4 can be low in the range but with a much higher in range T3 reading - and this is ok - and the blood test needs to be around 10-12 hours from your last dose and you need a fasting early morning blood test and just take water o/night until after the blood draw and then take your AM dose of the NDT.

No thyroid hormone replacement works well until the core strength vitamins and minerals - those of ferritin, folate, B12 and vitamin D are up and maintained at optimal levels -

I now aim to maintain my ferritin at around 100 - folate at around 20 - active B12 at around 125 ( serum B12 at around 500++ ) and vitamin D at around 125.

Your CR-P marker is in red signalling o/range high - though can't see the actual reading - and this is an inflammation marker showing your body under stress -

Your TSH is showing very high with your T4 and total T4 very low - showing under medication of thyroid hormone replacement - which we know has happened.

You have antibodies readings and these can be raised due to RAI and causing the high inflammation reading.

Your ferritin is very low - looks like 20 - suggesting iron deficiency anemia and this must be investigated before any supplement of iron tablets - my ferritin was down at 22 and I had to have a colonoscopy and endoscopy before I was prescribed iron tablets - not that they worked for me - but that's another story for another day.

RAI is known to ' trash ' vitamins and minera;s :

I think your B12 is ok - but the first thing is to get your ferritin up as all my research suggested

that no thyroid hormone replacement works well until ferritin is at least over 70 :

What is wrong with your eye- is this related and Thyroid Eye Disease ?

Poor you, you must feel terrible.

You definitely need to get back up to a stable replacement dose of thyroid hormone.

Dry mouth can be Sjogrens, maybe ask to rule that out. It’s common to have more than one autoimmune condition. Your raised TPO antibodies confirm autoimmune thyroid (Graves and or Hashimoto’s)

Mouth related issues can also be under replacement (scalloped swollen tongue etc)

I’d ignore B12 it’s good if it’s high.

Do you have a weekly pill dispenser? When I was very hypo and couldn’t think I found it helpful to have it all laid out with days of the week on and I knew if I’d taken something or not.

Thank you I used to have an alarm for my afternoon dose this was on my phone I need to put that back on .
