TSH 5.050

t3 3.45

t4 1.17

These are the results of my 32year old daughter whose doctor is saying she might need meds. What are your thoughts?

Have you got the reference intervals (ranges) for those tests? We can take a reasonable guess for TSH, but T4 and T4 need to be added. Especially as 1.17 does not seem feasible with standard ranges used in the UK. And we need to be sure they are Free T4 and Free T3 tests.

The reference intervals (ranges) depend on the lab - not the country!

Do you have the ranges?

Yes, she needs FULL thyroid retest in 6-8 weeks including thyroid antibodies

Meanwhile request GP test her vitamin D, folate, B12 and ferritin levels if not tested

Or add results if you have them

ALWAYS do thyroid testing early morning, ideally before 9am and only drinking water between waking and test

Presumably you have autoimmune thyroid disease ?

Autoimmune diseases often run in families as are very genetic

For full Thyroid evaluation she needs TSH, FT4 and FT3 tested

Also both TPO and TG thyroid antibodies tested at least once

Very important to test vitamin D, folate, ferritin and B12 at least once year minimum

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high TPO and/or high TG thyroid antibodies

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis.

Both are autoimmune and generally called Hashimoto’s.

Significant minority of Hashimoto’s patients only have high TG antibodies (thyroglobulin)

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins
