Hello,

I’m new to this community and was diagnosed with hypothyroidism in February, although I suspect I’ve been dealing with it for years. My initial bloodwork back in December 2017 only included TSH, but I’ve learned through this forum about the other essential tests. I appreciate all the valuable information shared here!

I’m looking for advice regarding my recent test results, which I’ll share below. My doctors believe I’m doing fine now that my TSH has dropped a bit, and they don’t see a need for adjustments aside from prescribing 50,000 IU of Vitamin D once a week for eight weeks, followed by 2,000 IU daily afterward. I’ve been on 25 mcg of Levothyroxine for just over two months. Thank you in advance for any insights!

Blood Work Results (from 4/20/18):

TSH: 4.23 mIU/L (Range: 0.40-4.50)

Previous TSH: 5.27 (Dec 2017)

Free T3: 2.8 (Range: 2.3-4.2 pg/mL)

Free T4: 1.2 ng/dL (Range: 0.8-1.8)

Total T4: 6.1 mcg/dL (Range: 4.5-12.0)

Thyroglobulin Antibodies: 293 (Normal: ≤ 1 IU/mL)

Thyroid Peroxidase Antibodies: 54 (Normal: < 9 IU/mL)

Vitamin B12: 482 pg/mL (Range: 200-1100)

Vitamin D, 25 Hydroxy: 23 ng/mL (Range: 30-100)

Folate: 17.4 ng/mL (no range provided, but I believe it’s low)

Ferritin: 158 ng/mL (Range: 20-288)

I’m eager to hear your thoughts and appreciate any guidance you can provide!

25mcg of Levothyroxine is a very low starter dose, usually reserved for the elderly or those with other health conditions. I would push for an increase to 50mcg daily and retest levels 6-8 weeks after being on a consistent higher dose. Likely you will need further increases, as TSH should always be under 2 (and most members report feeling best when TSH nearer 1)
