Good Afternoon

I am currently on 2.5 mg Carbimazole 3 times a week. Have 2 overproducing thyroid nodules and was diagnosed Hyperthyroid in 2019. Have been sent back to GP for monitoring as haven’t proceeded with treatment i.e RAI.

TSH 3.59 0.27 to 4.20

T4 15.4 12.0 to 22.0

T3 4.3 3.1 to 6.8

These are NHS Ranges

These are private bloods ranges

Vit D 83 50 to 200

B12 499 138 to 652

One out of range on private bloods

Kidney Glomerular Filtration Rate 75 ML/min1.732<90

Hi Hugoo

The most important thing.....How are you feeling?? the test results could 'look perfect' but if you are feeling well they are no good. Equally you could have bizare bloods and feeling the best you ever have. The symptoms or lack there of are a very essential piece of the jigsaw. Just because many doctors are not trained to consider symptoms does not make them irrelevant and if a doctor tries to callibrate the patient to the lab results rahter than using them as a guide, well, that is treating the labwork not the patient.

My heart rate is still high when walking. I was told I had thyroid induced Sinus tachycardia. Also feel palpitations at night. Tired and still anxious.

Having low iron and/or ferritin (iron stores) can cause tachycardia and palpitations. Having low levels of iron/ferritin is common with thyroid disease, both underactive and overactive.

Your doctor might agree to test your ferritin. But if you want to know more details about your iron you would need an iron panel. Getting the NHS to do one of those is a lot harder than getting just ferritin tested.

With a TSH of 3.59 you would feel better reducing your Carbi dose. Could you try dropping one of your doses or cutting one of your doses in half?

Yes I am going to try this as the doctor won’t test again until May now unless I get new symptoms

Hey there again

I would think with a TSH over 2 you are likely dealing with hypothyroid symptoms now.

How are you feeling ?

Could you consider reducing the AT drug a little - what dose are the tablets - or maybe skip a dose and see how you go ?

Do you have ferritin and folate readings ?

I think a RFA proceedure was suggested previously - is this an option worth considering ?

Hi pennyannie. Have palpitations and resting heart rate has increased in last couple of weeks. Always tired. My old Endocrinologist said RFA not an option.

Would you consider reducing your Carbimazole a little ?

With regards to the core strength vitamins and minerals suggest you aim for a ferritin at around 100 - folate around 20 - active B12 75++ ( serum B12 500++ ) and vitamin D around 100:

I don't know how old you are, but GFR does tend to decrease with age. You can get more info on GFR from this link :

GFR and kidney health can be improved with a good diet. I've discovered that sugar and processed carbs can have a bad effect on my GFR, and if I eat better my GFR will rise. Healthy fats and protein are not unhealthy for the kidneys.

Personally, I consider fats and oils that were eaten 200 years ago (i.e. weren't produced in a lab) as the healthy ones.

Untreated, under-treated or poorly treated thyroid can decrease kidney health.

I don't know what effect carbimazole has on kidneys.

Thank you. I am 59. I do eat more sugar than I should. My diet otherwise is very healthy. Plenty of veg,beans and pulses.

Where do you get your protein from? Beans and pulses don't contain complete proteins.

FT4: 15.4 pmol/l (Range 12 - 22)

Ft4 only 34.00% through range

FT3: 4.3 pmol/l (Range 3.1 - 6.8)

Ft3 only 32.43% through range

High heart rate can be due to being hypothyroid

Not everyone gets lower heart rate

Low GFR should improve as low Ft3 increases

The GFR is reversibly reduced (by about 40%) in more than 55% of adults with hypothyroidism[40] due to several reasons.

Thank you very much for this. The doctor will not test me now for another 6 months now unless I get the very hyper symptoms back again. Sometimes I just wonder if I should have treatment in the hope all comes out well the other side. I’m not very good at deviating from what I’ve been told to take.
