help please re 13 year old

my daughter has been unwell number of weeks- palpitations, nervousness, anxiety, nausea, dizzy, difficulty focusing, intolerance of heat, hair breakage, rashes the list is endless. Affecting her school and general well being. Normally very active, sociable. Had major knee surgery in Oct but recovered well and resilient throughout. End of March TSH 0.6 and t4 normal. Seemed to get better for a bit and symptoms generally improved then just over a week ago boom, worse than ever. Had bloods taken again and this time TSH dropped to 0.3, t4-19 and t3 normal. Discussed with endocrinology paed who said all ok and no need to retest again. I’m very frustrated and confused as this does not seem normal to me. I don’t know what to do next. Do we just wait until she gets more unwell. I can’t bear this. Or am I totally off the beaten track thinking it’s thyroid. Don’t want any advice on iodine etc if you don’t mind I am just at this stage trying to get a diagnosis to make my little girl feel better x

Has GP tested B12, folate, ferritin and vitamin D

B12 levels can drop significantly after aesthetic

Is she vegetarian or vegan or heavy periods

Hairloss often low iron/ferritin

Also needs thyroid antibodies tested

Hashimoto’s (autoimmune HYPOthyroid) frequently starts with transient hyperthyroid results and symptoms before becoming increasingly hypothyroid

Testing TPO and TG antibodies for Hashimoto’s (but can be slightly raised with Graves’ disease)

Testing TSI or Trab antibodies for Graves’ disease (autoimmune HYPERthyroid)

believe me I am trying my best to get her tests. The doc even said in the phone their gut is it’s Graves’ disease but they not testing again as the results are ok. It’s quite frustrating. She has had blood tests for everything under the sun and everything else is ok x

'OK' is a matter of opinion. It would probably be best if you could post those tests/results/ranges on here and let people have a look. We have great experience with interpreting blood test results - something that doctors are very bad at! :)

Vitamin D

Folate

Ferritin

B12

Blue Horizon will test children. You would need to pay extra for private blood draw. They have list of private clinics on website

Recommended that all thyroid blood tests early morning, ideally just before 9am

This gives highest TSH and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

Having had surgery in the last few months I would suggest getting an iron panel done would be a very good idea.

A standard iron panel often consists of the following tests, but they do vary :

Ferritin

Serum iron

Transferrin or TIBC (Total Iron Binding Capacity)

Transferrin Sauration Percentage

CRP (C-Reactive Protein)

Getting Vitamin B12 and Folate levels tested would be helpful too.

please add actual results

Within range is not necessarily adequate

Eg B12 range is typically 180-680. GP would say B12 results of 185 or 500 were both “fine”. But B12 at 185 would be far too low

Similarly ferritin…..often too low….but still within range
