High Pth and borderline calcium

My calcium was coincidentally checked last may and was 9.9(upper limit 10.4)

Now my calcium is 10.4 and pth 106(upper limit 88)

My tyroid usg was done three Times last 6 months and all seems normal. My blood tests and abdominal usg is normal( no Kidney problem) My Alp is 90(upper limit 107) phosphorus 3.49 (upper limit 4.9)

Tsh 1.54 and vitamin d 70

Dexa Shows ostopenia. I am 60 years old female .

I am so tired of worrying. Does anyone have the same condition? I do cooking, house errands and daily walks. Sometimes I do get tired but think it is normal for my age.

I have ibs if there is any connection.

As for thyroid

Just testing TSH is inadequate

For full Thyroid evaluation you need TSH, FT4 and FT3 tested 

Also both TPO and TG thyroid antibodies tested at least once 

Very important to test vitamin D, folate, ferritin and B12 at least once year minimum

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high thyroid antibodies 

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis. 

Both are autoimmune and generally called Hashimoto’s.

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease (Hashimoto’s or Ord’s thyroiditis)

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis 

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

all thyroid blood tests early morning, ideally just before 9am

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

anti tg 3 <1.3 (Upper limit 4.5)

So you still need

TSH, Ft4 and Ft3 tested together. Plus TG antibodies

NHS won’t test TG antibodies when TPO antibodies are negative, so would need to test privately

Need ferritin and folate levels tested too

B12 looks high, what supplements do you take

Is that just a separate B12 or vitamin B complex

If it’s B complex it contains biotin and need to stop this 3-7 days before test as biotin can falsely affect test results

Are you taking any vitamin D

If yes, how much

With suspected parathyroid issues you probably shouldn’t be taking vitamin D

The vitamin is only methyl B and I didnt know I have to stop vitamin D. Could it be raising my pth-I had COVID last march and then sort of started taking 20000 iu for the next 2 months once a week. Then continued with 5000 iu +100 K2 daily and then switched to 1000 iu daily.

Taking high dose vitamin D can result in High calcium levels

How much vitamin D were you taking when calcium and Pth were tested

You need vitamin D, calcium and PTH tested together when not taking vitamin D

Thank you. I will see endo in 2 weeks and will discuss. I do have the app-the arrow stops between unlikely and possible for PHPT.

hello Ley, I had the condition, calcium high and PTH high, first noticed it in 2017. Was put in watch and wait approach-no good. April3,2023 I finally had my surgery, removal of an adenoma of the parathyroid gland(s). I highly recommend the FB group parathyroid UK Action for change. Lots of most useful information. thank you, ongoing ! Less pains, less headache, slowly more energy ( I walk for 1hr, cycle for 3 hrs…) it will take time since I had this condition for so long. Thankfully no kidney stones.

Quick update: Ultrasound detected a 7.8 mm adenoma on one of my left parathyroid glands. The rest three okay. Surgeon said still too small and should get back in October with new pth and calcium results.
