Help understanding blood tests. Undermedicated or perimenopausal?

help understanding latest blood tests...on only 37mg of levothryoxine and symtoms worse than ever but blood tests (TSH) say am on too much. Has anyone had this? Consultant has said maybe I am also going through perimenopause (have just turned 40). latest bloods show:

TSH 0.01 mI (L 0.27 - 4.20mIU/L)

FREE T3- (RSV) 7.9 (3.1-6.8)

T4 26.9 pmol (L 11.00 - 21.20pmol/L)

Serum Ferritin 50UG (13-150)

Serum creatinine 47 umol/L 47.00 - 99.00umol

Anti-TPO 345 ( < 25 )

Anti-TG 179 H ( < 40 ) IU/mL SH

The ultrasound showed my voume as 2.9 and the thyroid gland is very shrunken and 'may atrophy'

FSH 9.5

Oestradiol 106

my consultant dioes not belive in T3 medication and won't perscribe. any help super appreciated

Are you now taking daily vitamin B complex and separate B12 supplements

No vitamin D test result?

Are you currently taking vitamin D supplement

If yes how much

if no test now

Test twice yearly via NHS private testing service when supplementing

Ferritin - aim to keep at least over 70 minimum,

Increasing iron rich foods in your diet

Have you had coeliac blood test done yet and are you now on strictly gluten free diet and dairy free diet

Thank you so much. yes I now take B12 and B complex but not taking vit D consistently. Not had coeliac test done yet. I have reduced gluten but haven't completely eliminated. Thank you again

This thyroid test has most likely caught a Hashimoto’s flare ……when excess thyroid hormones are released as thyroid comes under attack

After each attack the thyroid becomes a bit more damaged and you need increase in levels

but also

Has endocrinologist tested for Graves’ disease antibodies

TSI or Trab antibodies. It is possible, but rare to have Hashimoto’s and Graves’ disease at same time

Treatment then is block and replace

Block with Carbimazole

Replace with levothyroxine

Thanks so much. No I havent had these I will request last time. I wondered if it could be atrophic thyroidisits but my Dr disagreed. thank you
