My last bloods were in 14th Nov 2022 TSH as 0.70ml (range 0.27-4.4)
Im on Levo Throxine 75 and Liothyronine 50mg
I was put on Liothyronine as a request as I didn't feel Levo was doing anything. I really struggle with fatigue and weight gain even though I eat clean and workout 3-4 a week and have an active life in general. When starting it, it made me feel amazing and i could see i was so much less puffy in the face and everywhere really! Plus I could wake up in the morning.
The past 6 weeks Ive been struggling again so, I reached out to NHS endo team to ask for an increase and they have come back with this:-
"Firstly when is your next blood test?
The last TSH ( mid November) was nicely in the lower half of the normal range and therefore I cannot recommend an increase in the Levothyroxine unless your thyroid function has changed. We do not increase the dose of Liothyronine- that is a standard dose. "
Does anyone have any recommendations for my response with regard to getting an increase in medication?
obviously I will request another blood test and ask them to tell the GP to test for T3, T4 and TSH (because they only ever do TSH) But Im Really Struggling so any thoughts would be greatly appreciated...
Thanks In Advance.

I have been dairy free in the past and Im not completely off it but I have minimal dairy at times. however I do like the occasional dairy milk chocolate bar.
How much levothyroxine are you currently taking?
75mg I was on 125 and then aug I changed to 75 levo and 50 lio
Do you always get same brand levothyroxine at each prescription
yes, Its always been the same brand of levo = Teva since in have been on the 75
How much T3.
50mg
Are you splitting the dose?
No - I take it all at 7:30am and then wait 1 hour before any dairy products (like a cup of tea and breakfast)
Always test thyroid levels early morning, ideally just before 9am and last dose levothyroxine 24 hours before test Day before test ALWAYS split T3…..ideally as 3 doses spread through the day and last dose approx 8-12 hours before test
I have not been aware of splitting the T3 dose, as I get one pill a day... would you recommend with food then? equally breakfast lunch and dinner?
What vitamin supplements are you currently taking When were vitamin D, folate, ferritin and B12 levels last tested
My folic acid was extremely low and I was on prescribed supplements for that daily for 3 months and then they did another blood test to which I was in the normal range. So that has now stopped. B12 was in range the last time in July but in the lower range of 50.9 Ranges = (37.5 -150)
I am not on any supplements and my anti bodies are sky high but I have never had them reduce to a normal range since I knew about the hashimotos in 2017...so I assume there is nothing i can do there.
I have a full blood test booked in at the hospital on Friday morning, so I can update this post with the results when I know. I will also try and to get a vitamin blood test too...
Thank you for your in-depth reply SlowDragon its very appreciated!
With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a B12 supplement as well as a B Complex (to balance all the B vitamins) initially for first 2-4 months.
once your serum B12 is over 500 (or Active B12 level has reached 70), stop the B12 and just carry on with the B Complex.
Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule)
Thorne currently difficult and expensive to get (normally is approx £20)
If you want to try a different brand in the meantime, one with virtually identical doses of the ingredients, and bioavailable too, then take a look at Vitablossom Liposomal B Complex. Amazon sometimes has it branded Vitablossom but it's also available there branded as Yipmai, it's the same supplement
IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results
In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate methyl folate supplement and continue separate B12
Many, many Hashimoto’s patients need to supplement vitamin B complex and vitamin D continuously to maintain OPTIMAL vitamin levels
And sometimes separate B12 ongoing as well
What are your most recent vitamin D and ferritin results
Thank you SlowDragon , im sorry Ive made a silly mistake today my actual meds I am on at the moment is 75mg levo and 10mg lio so that may change things, I will also have to rectify with the email sent to the endo team today as previous post states...
As for the current reply...
Are you vegetarian or vegan?
No
What are your most recent vitamin D and ferritin results
Vit D - 115 Ranges 50-200
Ferritin - never been checked I dont think :/
I will look into the B12 as you have given me quite a lot of info there and my brain is struggling at the moment.
Yes I have tried and its not bad at all... but its just so expensive, I feel I am constantly battling with what's good for my body and what my bank balance allows.
