hi there just got bloods back and I’d be grateful for input as I’m at the enocrynolygist tomorrow. I have hashimotos and underactive thyroid.

I’m currently taking 25 mg liothyronine and 75mg levo.

Serum vit B12 (xe2pf) 324 ng/L [190-910]

Serum folate (42U5.) 6.1 ng/ml (3.3-99,999)

Serum tsh(xaEKV) <0.05 miu/L [0.35-5.5]

serum free T4(xaERr) 12pmol/L [9-23]

serum free triiodothyronine(XaERq) 8.5 pool/L [3.5-6.5]

plasma parathyroid hormone level (XaldU) 8.9 pool/L [1.95-8.49]

serum transferrin (XE2dx) 2.7g/L [2.5-3.8]

transferrin saturation index (XaERN) 13.3% [15-56]

It would help people comment if you said what treatment you are on and how you feel?

hi there, I wrote above what meds I’m on. I was feeling ok, but now don’t feel great . I have also never had the plasma parathyroid test before and it’s abnormal so that’s worrying me slightly

There's no Vit D result there. Parathyroid stuff is complicated so your Endo can explain that way better than me. It can* be linked to low vit D.

Your folate is too low and needs supplementing. If we have low vitamin levels we cant use our thyroid hormone properly + feeling bad from the deficiency itself. Thorne Basic B complex would get that up and help you feel better.

Is there a ferritin result? Your iron looks on the low side but I'm no expert on that at all.

With FT3 over range it does look like you're a tad over replaced.

thank you so much for this. My liothyronine is so hard to cut and I think three quarters of a tablet would be better, but 25 mg is smallest tablet. Do you think my levo should be slightly higher so 25 mg more , I take 75 mg at the moment

I'm not sure how NHS T3 prescriptions work, I get mine privately. There are 20mcg tablets and 5mcg capsules from what I've seen. Not ideal though to be switching brand as dose equivalents and all that.

FT4: 12 pmol/l (Range 9 - 23) 21.43%

I see people saying they feel at their best with FT4 50%+ of range. You should only change one thing at a time though.

my ferritin was last done in March (xe24r) 42ng/ml [30-400].

I get my liothyronine from Turkey, but my appointment is to see if I can get it on nhs since it’s now available on nhs) the 20 mcg would be great xxxx I’ve got my newspaper article to show him xxxx

Good luck, I hope he gives it to you.

If you work on ferritin and folate you would feel so much better anyway. Make sure you have iron rich foods in your diet regularly like chicken liver pate.

There are pill cutters that are almost perfection with the smallest most crumbly pills. Trust me, I can barely put a duck into a duck hole in a kindergarten puzzle.. And I do it to perfection.

Cutting T3 into 1/4’s

Either use a sharp craft scalpel or get a pill cutter to cut into 1/4’s

Try 3 x 1/4 tablets…..spread across the day. Trying 1/4 tablet waking, mid-afternoon and bedtime (or early evening if last dose keeps you awake)

Day before test last 1/4 tablet T3 approx 8-12 hours before test

Last dose levothyroxine 24 hours before test

Meanwhile working on improving low vitamin levels

What vitamin supplements are you currently taking

Folate to low

B12 too low

Iron/ferritin looks low …need full iron panel test via endocrinologist

Essential to test vitamin D too

Quite likely low

Also, if not already on strictly gluten free diet you need coeliac blood test done BEFORE trialing strictly gluten free diet

that’s fab thank you as always. I’ve written it all down to ask tomorrow. I did as you say above re my times of taking meds prior to my blood tests . If I split into 3 times a day do I have to not eat for two hours after taking it ?

no …30-45mins is fine

Even levothyroxine you only need wait an hour

Due to your elevated PTH result I hope your endo will test this again in conjunction with calcium and Vit.D tests, as this could possibly indicate a parathyroid issue. Parathyroid conditions are not linked to thyroid ones, and are often not sufficiently investigated, so just mention that result to your endo and keep an eye on those tests. Hope the appointment goes well.
