TSH too high, FT3 low. You are a poor converter and that's why you're still putting on weight. What you need is some T3 added to your levo.

Was test done early morning, ideally before 9am and last dose levothyroxine 24 hours before test
TSH is too high
Ft3 too low
Request 25mcg dose increase in levothyroxine as first step, see if this improves Ft3 without Ft4 going too high
Retest thyroid levels 6-8 weeks after any dose change or brand change in levothyroxine
What vitamin supplements are you currently taking ?
B12 is low
Are you currently taking vitamin B complex and separate B12?
I done test around 10am couldn't get earlier appointments. I planned my dosage a few days before, that I would be in that 24 hour frame. I didn't take meds on the morning. I am only on a vit D supplement, but like most Doctors don't seem concerned about results, my ferritin is always very high, I'm post menopausal. This is the first time I requested T3. Thanks for response
Low B12
supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) may be beneficial.
This can help keep all B vitamins in balance and will help improve B12 levels

B vitamins best taken after breakfast
Thorne Basic B or Jarrow B Right are recommended options that contains folate, but both are large capsules. (You can tip powder out if can’t swallow capsule)
IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a B12 supplement as well as a B Complex (to balance all the B vitamins) initially for first 2-4 months, then once your serum B12 is over 500 (or Active B12 level has reached 70), stop the B12 and just carry on with the B Complex.

Hello Sallina :
The accepted conversion ratio when on T4 only medication is said to be 1 / 3.50 - 4.50 -T3/T4 with most people feeling at their best when they come in at around 4 or under.
So to find your conversion ratio you simply divide your T3 into your T4 and I'm getting yours coming in at around 4.27 so a little wide of centre.
Your T4 is at around 85% and your T3 at around 51% through the ranges and we generally feel at our best when both these vital hormones are up in and in the top quadrants of the ranges.
Your TSH is too high, signally a dose increase and there is room for a small increase in T4 and that might be all you need to push your conversion of T3 up into the 5's where I know I felt much improved.
There are some people who need a T4 slightly over range to restore their metabolism and I think the first solution is a dose increase regarding your question on weigh gain.
You may find your conversion improves a little with optimum vitamins and minerals - I now aim for a ferritin of around 100 : folate at around 20 : B12 500 ++ and vitamin D at around 100 :
Your ferritin is over range - do you know this - a question there for your doctor ?
