Suggestions on my results please

Can anyone look at my results and advise what I should be asking my endo next week please? I generally still feel unwell despite starting T3 last September. I thought I would be improving by now and my blood results aren't much better either. I'm aching, tired but not sleeping well, struggling with weight. Ectopic heartbeats settled this last week which is typical when I have a 48 hour monitor booked for 1st April. I'm tired of fighting this battle.

Finger prick bloods, fasting, 24 hrs post Levo and 9 hrs post T3. I take 75mcg levo and 10mcg liothyronine a day. Levo was increased early Feb from 75cg on 3 days, 50mcg 4 days a week, T3 the same as I struggle with ectopic heartbeats.

FT3 3.8 (3.1-6.8) was 4.2 end January.

FT4 12 (12-22) was 11.5

Tsh 0.06 (0.27-4.2) was 0.66.

I'm really despondent and feel I should be improving by now, considering changing endo, he's private but don't want to take the risk of wasting more money.

I'd suggest you need to increase lio - you're less than 20% through the free T3 range. Add another 5 mcg now and a further 5 mcg (ie to 20 mcg altogether) in 2 weeks, then hold for 8 weeks and re-test bloods. Try it in 3 doses a day to minimise heart issues

And if this doesn't work, or work enough, add another 25 mcg of levo a day after that x

Thank you for replying, I do feel I need more T3, I asked last appointment and he said no, increase levo instead. I'll tell him next week that's what I'm going to try, I can't keep this up. I was started on 5mcg T3 3 x day but because of the ectopics he said reduce to 2 x day. It didn't make much difference, I still had them so maybe they're anxiety related, not T3 but I feel I've gradually got worse not better.

So last 5mcg dose T3 was 9 hours before test?

Which brand of levothyroxine are you currently taking

Do you always get same brand levothyroxine

You probably need both levothyroxine and T3 increased but should only increase one or the other not both

Perhaps try increasing levothyroxine first up to 87.5mcg daily. This should increase Ft4 and Ft3

What vitamin supplements are you currently taking

What are your most recent vitamin D, folate, ferritin and B12 results

Thank you so much, I'm on Eltroxin, have been for a few years now. I take vit D3 2,000 daily and magnesium. I stopped the B complex as gp tested my B12 in early January but I'd only been off it 5days so probably not a true reading and didn't restart it. I don't take vit K2 as I'm on blood thinners but I can't see that it's contraindicated. B12 was 600 (180-640), vit D 79.9 (>50). I don't know whether he'll increase my T3 or as you say increase levo next week. All I do know is I'm tired of not being well, probably because till 2018 I had just been ticking over nicely on levo only so I don't do not being busy, happy laughing me.
