Help with thyroid blood tests

Good afternoon everyone ❤️

I was wondering if anyone could help with these thyroid results, ( these results are for my cousin who's been suffering with overactive thyroid symptoms since early last year).. TSH... (0.25)...RANGE..(0.30..4.50)...T3..(4.6.)..RANGE..( 3.1..6.8).. T4.. (19.1)..RANGE..(11.00..22.00)..in the letter the endo notes a multinodula goiter on right side, subclinical thyrotoxicosis, an ultrasound scan has been made for her the letter ends.. If scan and thyroid bloods are satisfactory she will be discharged back to doctors care. She has been ill since last year and is struggling to hold down her job.. She is terrified the endo will just discharge her back to gp care.. No anti thyroid drugs have been given.. Also she had a discussion with the endo about the worst case scenario, the endo only suggested radioactive iodine therapy, I've spoken to my cousin and asked her to look up every option that's best for her, don't be bullied in to a treatment you don't want.. Look what's best for you.. I was thyrotoxic (graves) and my surgeon ruled out radioactive iodine therapy, but we're all different I suppose, anyway thank you to anyone who respondes 👍

Has she had thyroid antibodies tested for both Graves and Hashimoto's?

TSI and TRAb for Graves.

TPO and Tg for Hashimoto's.

Although her TSH is very slightly under range, her FT4 and FT3 are not suggesting overactive.

Does she have any other test results for thyroid or just this one set?

Hi slowdragon ❤️Thanks for the reply.. She did have another set which again showed just under range TSH.. T3 in normal range T4 top end I think.. But this is how I started with my thyroid illness 2 short suppressed TSH.. 2014 normal T3, T4 top end.. Again in 2015 short supressed TSH.. Endo said auto immune thyroiditis (I never found this out till last year in my medical records) my cousin is presenting with the same symptoms I did.. Sweating, tremors, insomnia, hair loss, fast heartbeat, high blood pressure, she can't sit still and she says she feels hyper wants to sleep but is finding it impossible,

Sounds more like Hashi's then. Need to get those antibodies tested, definitely no anti-thyroid drugs unless TSI or TRAb antibodies are positive.

Hi seasideSusie❤️Hey got Yr name right.. 👏.. Just wanted to touch base on the antibodies I've asked her about them she says she's been tested and they were negative 🤷‍♀️.. I had mine done to although I never knew I had, I found them in my medical records I requested last year.. 2015..antibodies test negative, but in 2014 the endo diagnosed me with auto immune thyroiditis, again I never new until I read through my records,🤷‍♀️ can I pick your very knowledgeable brain? .. Given she diagnosed me with auto immune thyroiditis does that mean their was something going on with my thyroid all those years ago? Because when I pushed my gp in 2016 about my awful symptoms which were hyperthyroidism he said my thyroid was fine I'd never had anything wrong with my thyroid, well he was wrong as I lost it in 2019 to graves thyrotoxicosis, I has 2 short suppressed TSH but in rang T3 top end T4 in 2014..then again short supressed TSH in 2015..again T3 normal and I think T4 top end.. I'm trying to gather info on all this as I'm going to practice manager with my complaint of miss diagnoses of hyperthyroidism, many thanks susie❤️❤️❤️

You can apparently have Hashi's without raised antibodies (just to complicate things, as if it wasn't difficult enough 🙄).

I don't know anything about Graves other than you need raised TSI or TRAb antibodies to confirm it.

I would ask you friend to find out exactly which antibodies were tested. Ideally it would be all four that I've listed above. Antibodies tested at GP surgery tends to only be TPO, I think it's usually an endo who will test the others.

Sorry seasidesusie ❤️Got Yr name wrong, though I was replying to slowdragon, I have terrible brain fog at the min going through my own hell with my parathyroid glands and calcium going up.. I'm trying to help her as she's going out of her mind.. 🙄

Looking more like Hashimoto’s possibly than Graves

Has endocrinologist tested thyroid antibodies

TSI or Trab antibodies for Graves’ disease

TPO and TG antibodies for Hashimoto’s

What medication is she on…

ESSENTIAL to test all thyroid antibodies and vitamin D, folate, ferritin and B12

Suggest she get FULL thyroid and vitamins tested BEFORE consultation

Low vitamin levels are extremely common with Hashimoto’s or Graves

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

Hi slowdragon ❤️Thanks for the reply, she's had 2 TSH done both were below range.. But T3 in range T4 at top end.. But I was the same 2 Short supressed TSH.. In 2014 and 2015 endo said auto immune thyroiditis ( I've just found this diagnosis in my medical records last year I had no idea I had it because when I started to tell my gp I was sure my problem was my thyroid he said you have nothing wrong with Yr thyroid never have🤷‍♀️.. She as not been offered any antibodie tests, only the scan.. Your probably right it could be hashimos, but she's not been hypo.. She's lost over 2 stone in weight to..

Hashimoto’s frequently starts with transient hyperthyroid results and symptoms before becoming increasingly hypothyroid

A scan might show a toxic “hot nodule”

Come back with new post once she gets full test results

Only do private testing early Monday or Tuesday morning and then post back via tracked postal service

P,S, I'm confused as these blood test results are not looking " hyper " at all :

If Graves was diagnosed this is said to be life threatening if not medicated so I doubt this and suggest it is likely the goitre causing her erratic thyroid hormone production and symptoms.

Hi pennyannie❤️This is how I started with my thyroid problem, I had several suppressed TSH but T3 in range T4 top end my gp never addressed the situation until I became hyperthyroid in 2018 but had symptoms from 2014 onwards, I was told T4 at top end TSH under range was subclinical hyperthyroidism, my blood came bk in 2018 as TSH... 0.02...T3 24.2..no T4 done until next 7 weeks it was 100..i reckon my thyroid was failing as early as 2015 as I had overactive symptoms, my cousin is presenting with sweating, tremor, insomnia, fast heartbeat, hair loss weight loss, been in hospital with hight blood pressure fast heartbeat, thanks for the info.. 👍
