Updated blood test results. TSH high.

I have some updated blood test results, any advice would be welcome.

TSH( 4.13 mul)(0.35-4.94mul) or (0.5-2.5mu/l) as I have no thyriod

FT4 (13.8pmol/l) (9-19pmol/l)

T3 (3.6. pmol/l)( 2.6-5.7pmol/l (wasn't tested this time) I did mention it to the nurse she stated its what the doctor has wrote down.

These results were requested by my doctor for a health review, maybe why he didn't requst T3. My surgeon usaully requests my bloods and checks them, I'm not sure he will see thease as they've gone back to doctor. I'm frustrated as I can't get through to the doctors surgery, I'm not due to see my surgeon until late January.

I've starting taking medication at night time, I than feel better during the day, maybe this isn't a good idea looking at my TSH results

I've started taking omega3 and vitimin D 3,6,9 again after a 10mnth break from them.

I'm feeling better, not suffering with insomnia. However I'm still fatigued during the day. also putting on weight hair thinning.

I have sourced my own T3 and considering if/when to start taking a small dose.

Any advice would be welcome

Was test done as early as possible in morning before eating or drinking anything other than water and last dose levothyroxine 24 hours before test

How much levothyroxine are you currently taking

Do you always get same brand levothyroxine at each prescription

Are you saying your

TSH is 4.13

Ft4 13.8 (9-19)

Your results suggest you are under medicated

Ft4 is only 48% through range

Essential to test vitamin D, folate, ferritin and B12 too, especially when under medicated

What vitamin supplements are you currently taking

On levothyroxine TSH should ALWAYS be under 2

Most people when adequately treated will have TSH around or under one, Ft4 at least 50-60% through range

Ive never stopped my levothyroxine before testing as I have no thyriod and have not been advised by my surgeon to do this. However there was 6hrs between.

I'm shocked at how much my TSH has got up in the last few weeks, also my FT4 has been slowly dropping.

I'm taking vitimin D also Omega 3 6, 9

I always get the same brand of levothyroxine as it suits me better, I've recently started taking levothyroxine at night time.

I might ask for my blood to be retested and my T3 to be tested this time. I'm concerned that I can't just do nothing,

I'm not seeing my surgeon until January. The doctor and encrinologist previously wouldn't change my medication.

Any advice would be appreciated.

How much levothyroxine are you currently taking

Which brand is it that suits you best

Levothyroxine is storage hormone so you can adjust timing of dose before test

As you normally take levothyroxine at bedtime/in night ...adjust timings as follows just prior to blood test

If testing Monday morning, delay Saturday evening dose levothyroxine until Sunday morning 9am

Then test Monday morning 9am (this gives 24 hour gap between last dose and test)

Delay Sunday evening dose levothyroxine until immediately after blood test on Monday morning. Take Monday evening dose levothyroxine as per normal

Taking levothyroxine only 6 hours before test give false high Ft4 result…….so your even more under medicated than this result suggests…..as confirmed by high TSH

When you take levothyroxine at bedtime do you always take on empty stomach and no other medications or supplements within at least 2 hours

How much do you weigh in kilo

Guidelines on dose levothyroxine by weight is approx 1.6mcg per kilo per day…….but some people need higher dose

Yes always on an empty stomach, I'm on 75mcg and I'm 82kg. Which is under medicated by the rule of 1.6

Maybe it's time to try again with 100mcg?.

I was taking 100mcg my TSH was 0.03 and had symptoms of being over medicated sweating insomnia I could feel the my pulse in neck throbbing at night time. I couldn't find words or form sentances.

My medication brand was changed, I'm now on teva brand which suits me better.

However my operation was in February and I had my last covid jab in may. Maybe my body was trying to adjust to everything.

I've phoned too asked if there is any earlier appointments or a telephone call advice.

I'm hoping tommorow will bring some postive news.
