Hi new here would really appreciate some help with results

Hi can anyone give me advice pls, Judy had bloods done as suffering various symptoms for few years now but doctors not willing to help much, I’m a member of a few groups and have said could be possibly hypo from these results, would really appreciate and adduce thank you

Can you please add the reference ranges for the results. Ranges vary from lab to lab so we need the ranges that come with the results. If you have a print out the results will probably be at the side of the results.

Also, please add the units of measurement for Vit D and B12.

Hi thanks for helping, b12 was 197 range is 120 to 999 vit d was 60 range 50 to 150 folate was 4.3 range 2.5 to 999 tsh range o.3 to 5.0 t4 7.9 to 16.0

What’s your diet like

Are you vegetarian or vegan

Are you currently taking any other medications, eg Gaviscon, omeprazole or similar

Hi no not vegan just follow a fairly standard diet but will admit it’s not the healthiest, been bad for junk food the last 6 months and under eating slightly as not much appetite, ferrtin was ok at 88, I’m not taking any meds or supplements at all at the moment As only just got these results back, I’ve just paid for further thyroid testing and testosterone as was low in that few years ago

Ferritin at 88 is good for female…..but would expect higher if male

Also what age approx ….

Suggest you request cholesterol and testosterone tested via GP …or privately

I’ve just ordered a full thyroid panel, active b12 and full testosterone from medivet should be here Monday, my doctors refuse to do anymore tests and won’t help with anything as saying everything in range 🤷‍♂️

Thyroid tests should be done no later than 9am to get the highest possible TSH which is needed for diagnosis. It's also best not to eat or drink anything except water before the test because some foods and drink may affect TSH. Also we should not take any supplement containing biotin (eg B Complex) for 3-7 days before any blood test as this gives false results. Is this how you did your test? Assuming you did then:

Tsh 2.36 (0.3-5.0)

T4 11.6 (7.9-16.0)

These results are classed as euthyroid (normal). A normal healthy person would have a TSH of no more than 2, often around 1 - see graph here:

and an FT4 around mid-range-ish

Your TSH is a little higher, it may be normal for you but it may also be indicating that your thyroid is starting to struggle. Your FT4 is 46% through range so again that may be your normal level and it may be on the low side.

You really could do with having TPO and Tg antibodies testing to see whether there is any suggestion of autoimmune thyroid disease (Hashimoto's) which is the most common cause of hypothyroidism and is where the immune system attacks and gradually destroys the thyroid.

Primary Hypothyroidism is diagnosed when TSH goes over 10 and Autoimmune is diagnosed when TSH goes over range but not necessarily as high as 10 but with raised antibodies at the same time.

B12 192 (120-999)

I believe this is a big problem for you. I think the unit of measurement is ng/L or pg/ml (they are both the same) but please correct me if I am wrong.

If I am correct then you could be looking at B12 deficiency. Do you have any signs of B12 deficiency – check here:

If you do then list them to discuss with your GP and ask for testing for B12 deficiency and Pernicious Anaemia. Do not take any B12 supplements or folic acid/folate/B Complex supplements before further testing of B12 as this will mask signs of B12 deficiency and skew results.

Please come back and say if you have any of the signs or symptoms for further suggestions.

Folate 4.3 (2.5-999)

This very low although that range is basically impossible, nobody would have a folate level of 999.

Your level is not folate deficiency but according to NICE it is suggestive of deficiency - see

Folate level

◦Serum folate of less than 7 nanomol/L (3 micrograms/L) is used as a guide to indicate folate deficiency.

◦However, there is an indeterminate zone with folate levels of 7–10 nanomol/L (3–4.5 micrograms/L), so low folate should be interpreted as suggestive of deficiency and not diagnostic.

You may wish to discuss this with your GP.

Do not start taking folic acid or a folate supplement until after further testing of B12 and injections or supplements started.

Vit d 60nmol/L

This is low but no low enough for your GP to treat so this is something to address yourself.

The Vit D Society and Grassroots Health recommend a level of 100-150nmol/L, with a recent blog post on Grassroots Health mentioning a study which recommends over 125nmol/L.

To reach the recommended level from your current level, you could supplement with 4,000iu D3 daily.

Retest after 3 months.

Once you've reached the recommended level then a maintenance dose will be needed to keep it there, which may be 2000iu daily, maybe more or less, maybe less in summer than winter, it's trial and error so it's recommended to retest once or twice a year to keep within the recommended range. This can be done with a private fingerprick blood spot test with an NHS lab which offers this test to the general public:

Doctors don't know, because they're not taught much about nutrients, but there are important cofactors needed when taking D3. You will have to buy these yourself.

D3 aids absorption of calcium from food and Vit K2-MK7 directs the calcium to bones and teeth where it is needed and away from arteries and soft tissues where it can be deposited and cause problems such as hardening of the arteries, kidney stones, etc.

D3 and K2 are fat soluble so should be taken with the fattiest meal of the day, D3 four hours away from thyroid meds if taking D3 as tablets/capsules/softgels, no necessity if using an oral spray.

For D3 I like Doctor's Best D3 softgels, they are an oil based very small softgel which contains just two ingredients - D3 and extra virgin olive oil, a good quality, nice clean supplement which is budget friendly. Some people like BetterYou oral spray but this contains a lot of excipients and works out more expensive.

For Vit K2-MK7 I like Vitabay or Vegavero brands which contain the correct form of K2-MK7 - the "All Trans" form rather than the "Cis" form. The All Trans form is the bioactive form, a bit like methylfolate is the bioactive form of folic acid.

Magnesium helps D3 to work. We need magnesium so that the body utilises D3, it's required to convert Vit D into it's active form, and large doses of D3 can induce depletion of magnesium. So it's important we ensure we take magnesium when supplementing with D3.

Magnesium comes in different forms, check to see which would suit you best and as it's calming it's best taken in the evening, four hours away from thyroid meds if taking magnesium as tablets/capsules, no necessity if using topical forms of magnesium.

So at the moment I think your biggest problem is B12 and folate and I'd work on getting them to optimal levels - B12 at least 550pg/ml (or ng/L) but even better would be around 900, and folate into double figures. Come back and let us know what your GP says about these results. Also you should improve your Vit D level.

It would be helpful to have ferritin tested and if this is less than 30ug/L this confirms iron deficiency according to NICE, in which case you'd need an iron panel and a full blood count to confirm iron deficiency and to see if you have anaemia.

Don't start all supplements at once. Start with one, give it a week or two and if no adverse reaction then add the next one. Again, wait a week or two and if no adverse reaction add the next one. Continue like this. If you do have any adverse reaction then you will know what caused it.

Thanks for all that information, I do fit a lot of symptoms for b12 deficiency, muscle pain, low energy, severe anxiety, depression, forgetfulness etc, I have just paid for some more bloods to be done and should be here in the next couple days so by the end of next week will hopefully no more, my ferrtin was ok at 88! I’ll get these other bloods checked first and then start getting a protocol in place I think

Can you add the ranges for those results. Lab ranges vary and to be accurate we would need them.

Most have a TSH of around 1. but the range is much wider and GPs don’t diagnose a issue until it reaches 10.

Your FT4 looks low by most ranges.

What you need to investigate further is FT3 result and ferritin and thyroid antibodies TPO & TGAb.

Not sure picture is clear.. tsh range 0.3 to 5.0 t4 range 7.9 to 16.0... my doctors really don’t like doing blood tests for me it’s like they can’t be bothered, I’ll have to pay to have some private tests done

Sorry your doctors are unhelpful it sounds worrying when doctors can’t be bothered to do blood test. What are doctors putting symptoms down to?

Often when TSH is in range the lab does not test FT4.

Your FT4 11.6 [7.9-16] is 45% into range which might be normal for you, do you have any previous results yo compare. Also you have no FT3 result and this shows the active hormone levels.

Some of your nutrients look low too, in range is not same as optimal.

Arranging a private blood test might be good idea. Some companies offer packages including full thyroid function including thyroid antibodies (& nutrients) which might be a good idea to rule out potential thyroid issues.
