Here are my latest blood results through Medichecks. I’m pretty surprised as I had been on a very strict detox/clean way of eating. I take 18.25 of T3 daily and 50/75 mcg Levothyroxine (alternate days) on the advice of my Endo since starting T3 last October. I’m guessing I need to increase either T4 or T3 or both?
My B12, Folate, Ferritin, vitamin D are all in range.
I do get eptopic heartbeats regularly, but they might be caused by on going stress which I have looking after my disabled daughter.
TSH - 0.06 (0.27-4.2)
T4 - 10.9 (12-22)
T3 - 3.1 (3.1-6.8)
Cholesterol total 7.5 <5
LDL 4.9 <3
Non HDL 5.9 <3.8
HDL 1.6 >1.2
ALP 130 (35-104)
Haematocrit 0.465 (0.33-0.45)
I would appreciate any help you can give me.

How much levothyroxine were you taking BEFORE T3 was added
Do you split the T3 as 3 x 6.25mcg
Was this test done as early as possible in morning before eating or drinking anything other than water and last dose levothyroxine 24 hours before test
Day before test did you split T3
Last 1/4 tablet T3 approx 8-12 hours before test
How do you feel
Suggest you request 25mcg dose increase in levothyroxine
This should increase both Ft4 and Ft3
Retest in 6-8 weeks

Do you supplement iron as well as b12? Or is your diet high in iron rich foods?
I bet you feel pants. Everyone is different but that is a low t3 for the amount your taking. Do you take it with food or on an empty stomach?
If those results were as Slowdragon mentioned and were early morning, before food and you took your levo and lio as per opinion on here, then I would raise my levo first. I see how I felt in 6 weeks and take bloods. Personally I find levo easier to take than lio and am sure (not certain yet though admittedly) that t4 helps with my main symptoms but lio definitely helps with my energy but is less stable. You understand this maybe just me though.
You obviously have a lot on your plate and I’m sorry that you must be feeling unwell when you’re trying to deal with stress too but I would also make sure, as Lalatoot said, that you are eating enough. Is your clean/restrictive diet low carb? I know this suits some but it made me quite poorly. I need to raise my carb intake amd it still took me 5 months to feel better. Including having a menstrual cycle again.
Your results are showing you are under medicated, further evidenced by high cholesterol & liver enzymes which are classic hypo symptoms. Also ectopic heartbeats.
FT4 is under range & FT3 right at the bottom of range. How are you feeling? When are you due to see/correspond with your endo?
Eating clean is great as should have gone towards reducing antibodies further, and as helps with improved liver function should help with better thyroid hormone conversion.
Hi that would definitely make sense that being under medicated affected cholesterol and liver & heart as in my case. A big ask….but do you know or understand the science behind why being low on thyroid hormone can affect these things? Would be good to understand. Thanks
The high cholesterol & raised ALP are to do with liver function slowing down with inadequate thyroid hormones levels. Also the gall bladder which metabolises liver byproducts slows down, so too the flow of bile used to breakdown fats and cholesterol and other toxins. Nothing is working efficiently & fats/toxins build.
Also, don’t forget that a huge amount of T4 to T3 conversion takes place in the liver and so when it is all gummed up with cholesterol, etc, it fails dramatically in doing a good conversion job.
With regard to the heart issues & low thyroid hormone, tattybogle found this link that I have saved, as my own heart related experiences are forever embedded in my memories because they were so frightening and avoidable had I been given adequate thyroid hormone replacement meds!

Yes it does look like you’d benefit from a good increase in both t4 (id go back to 100 or 125 really) - this business of reducing t4 when you add in T3 I’ve found to be madness - and T3 (25?)I certainly didn’t feel well till my T3 was over 6. People often seem to blame it on stress when they’re under medicated. I do think you’ll be able to cope much better when you feel more yourself.
Thank you all for your replies, all very helpful. I have increased my Levo up to 100mcg and may even go back to 125mcg but will stick with this for now, then get re tested in 6 weeks.
