Hi I would just like to ask about my recent Medichecks blood test.

20 April :

TSH: 3.82 (0.27-4.2)

T3: 2.69 (3.1-6.8)

T4: 10.8 (12-22)

I had bloods done at GP on 12 December but they only do TSH and this was 0.13 (0.10-4.00)

My last bloods done by Medichecks before that was on the

24 September 2019

TSH: 0.915 (0.27-4.2)

T3: 3 (3.1-6.8)

T4: 16.6 (12-22)

So obviously I need to up my meds. I’ve felt crap for months and my GP is clueless and leaves it to me basically. Anyway I’m on 50mcg of thyroxine so should I put that up to 75mcg? And I self source T3 but I’ve only been taking 6.5mcg but since I’ve been taking that my T3 has gone lower. Obviously with the COVID situation the best I could get would be a telephone consultation. Can I have some of advice please guys 🙏🙏🙏

Concentrate on raising your T3 first, that is the active hormone. It's not a good idea to raise both levo and T3 doses at the same time, so leave the levo for the moment, and increase the T3 by 6.25 mcg. You can do that every two weeks until you get to 25 mcg. Then, hold for six weeks and retest. :)

Thankyou. So take half a 25mcg tablet in the morning with the 50mcg thyroxine. I meant to add on the sept and December blood test I was on 75mcg of thyroxine but I lowered it myself because of palpitations. I then realised that was probably my hormones because I also had those tested and they were very low too even though I was on HRT. The GP actually wanted me to put my T4 up to 87.5mg after the sept test but the chemist couldn’t get hold of the 12.5 tablets so o left it at 75mcg. I went down to 50mcg in January because of palpitations x

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .

Last dose of Levothyroxine 24 hours prior to blood test. (taking delayed dose immediately after blood draw).

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, best not mentioned to GP or phlebotomist)

If/when also on T3, make sure to take last third or half of daily dose 8-12 hours prior to test, even if this means adjusting time or splitting of dose day before test

Is this how you do your tests?

Looking at results from 7 months ago when you were on 100mcg levothyroxine...your Ft4 was only 46% through range

Most people on levothyroxine need Ft4 at least 70% through range, often higher

You were not on high enough dose of levothyroxine and probably didn’t need to reduce levothyroxine at all and certainly not down to 50mcg

Personally I would increase levothyroxine up to 75mcg first and retest in 6-8 weeks

What vitamin supplements are you currently taking?

Levels were too low in Medichecks test

I take vitamin D 1000..so I take 75mcg thyroxine and 12mcg T3. Yes my tests were done as per requirements. If you can recommend any other supplements I’d be great full 👍 x thanks Slowdragon

Absolutely essential to regularly test Vitamin D, B12, folate and ferritin

1000iu is unlikely a high enough dose

GP will only prescribe to bring levels to 50nmol.

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

Vitamin D mouth spray by Better You is very effective as it avoids poor gut function. There’s a version made that also contains vitamin K2 Mk7

It’s trial and error what dose we need, we frequently need higher dose than average

Low B12 can cause pins and needles

Ask GP to test B12, folate, ferritin and iron
