Low free T4 on NDT what to do?

So I have been on 1 grain of Thyroid-S for 8 months or so now and according to my last 3 blood tests my levels have stabilised as they have all been very similar.

TSH. - 2.8

Free T4 - 9.0

Free T3 - 5.2

Ranges where all slightly different but nothing from the norm. Free T4 is always flagged as 10-15% under range.

I did try 6 weeks of 1.5g at the start of the year and while that pushed TSH lower it did nothing to budge either T4 or T3 or make me feel particularly good

I have read some reports that I am never going to feel quite 'right' until my T4 is well in range but I am not sure how I can achieve this. Levothyroxine is not an option as it makes my joints feel like a 70yr olds inside of 3 weeks.

How to move T4 up?

There is no norm where ranges are concerned. Can you please add the ranges so that your results can be interpreted and suggestions made which may help.

SeasideSusie is quite right, we need to see your results with the ranges. The ranges differ from lab to lab so without seeing yours we should just be guessing which won’t help you at all.

I’m also on Thyroid S so just want to pass on a few things my Endo told me which may help. When correctly medicated on NDT TSH is usually suppressed, 0.01 is not unusual, FT4 can fall in the range as if we are on the correct dose for us then FT4 isn’t always needed if we are getting enough T3 so I always think of the T4 as a sort of back up. But FT3 should be in the top quarter but never over and the FT3 reading is the only accurate reading. May be when you added more in then it was too much for your body at that time or maybe something in the fillers isn’t agreeing with you and Thyroid S is high on fillers. My earlier research when I came off Levo showed that Thyroid S and Thiroyd were not interchangeable but I was concerned about the numerous fillers so I went over to Thiroyd and tried that and I was so poorly so I went back to the Thyroid S but you be fair I hadn’t given the Thiroyd a fair trial as I didn’t take it long enough to do bloods and I didn’t try varying the dose to see if I needed more or less but as I’d felt good on the Thyroid S I stuck with it and feel very good on it although my now so called expert isn’t liking my suppressed TSH so isn’t familiar with what I’m taking or what the results should be like. They won’t believe that one of their then colleagues from Romania and used to NDT actually got my dosing correct for me but at least I’m told I know and understand what I’m doing! Since then I’ve found that the S stands for slow release and it certainly works for me like that. Years ago the NHS started my thyroid journey on NDT but I was never tested back then. I was-started on a grain and after a longish time wasn’t feeling as good so no testing but given another grain and again fine. ( the body) takes the amount of T3 that it needs). It was a long protracted strike in Canada where the medication came from that meant I went over to Levo and to be fair I was good with that as well.

We shouldn’t really compared different medications but as a very rough guide I was taking 125/120 mcg Levo and now take 1.75/1.75/1.50 over three days rotation. I’ve also been told that the same amount of NDT should be taken daily but I juggle a little bit cutting them in an attempt the even then out more and I am not aware the amount is lower on one day. I’ve always taken all my Thyroid S in the morning as I kept forgetting the second dose but now I know it is slow release then that convinces me I’m doing the right thing taking it all at once and I’m consistent with the readings the real expert had me on but be aware this is my story so yours could work out differently. So please post your ranges and I’m sure you will get more feedback then. But always listen to your body and work together.

TSH 2.6 - range. .27 - 4.2

FT4 9.4. - range 12 - 22

FT3. 5.2. - range 3.1 - 6.8

Thanks for the replies! maybe I need to try an increase again. It didn't go well last time but maybe there were other factors

Your FT4 is 26% under the range, and FT3 is 56.76% through the range. Yes, you are undermedicated. Try the 1/2 grain increase again, but give it a good 6 weeks before you test bloods again It might not be enough of a bump.
