I started on 1/2 grain and I used a pill cutter to cut the tablets. After 6 weeks I increased by 1/4 grain and waited two weeks. I felt fine so I then increased to 1 grain (1 tablet) With NDT you go by how you feel and if you feel jittery you can always reduce your amount by 1/4 grain. Others will be along to advise you.
I'd start with one grain of NDt which has the effect (approx) of 100mcg of levothyroxine and read the following link which is by one of Thyroiduk's Advisers (now deceased). You should increase according to how you 'feel' with the dose you are now on. You will find it helpful:
We are all individuals and can react differently but I’m sure we can help you on your journey. My main advice is to not increase too quickly and stay on each dose increase until you can feel the difference. Then stay on that dose long enough to recognise if it falls again. That tends to show it is ready for another increase. A quarter tablet though can make a huge difference. When you are in a stable dose and feel good then best to check levels and remember you need a ful panel as it’s most important that you test T3 as that will be the only accurate reading. Results are read differently if on any form of T3 so idea readings when properly medicated should be a suppressed TSHin the region 0.01 or 0.02, FT4 can fall in its range but FT3 should be in the top quarter but never over.
I mumissed my sweet spot doing this as I followed instructions to increase in half grains up to two and then go up in quarters but I got up to three grains and realised I wasn’t really improving so most likely overmedicated. But I had chosen when to start this as I had my Endo appointment already booked so I would at least have his advice and testing to sort me out if I got unstuck. I was put on NDT for about 5 years when I was first diagnosed getting onto 40 years ago now so my experience from the helped and I did correctly know I was overmedicated. Endo dropped me down to 1.75 which seemed a lot but it was spot on but going up in half grains to 2 grains meant I’d missed my sweet spot. As a very rough guide I was on 125/100 Levo before.
I’ve seen suggestions that the ‘S’ in Thyroid S stands for slow release and it certainly works like that. Early on I stopped splitting my dose as I kept forgetting the second and I thought by then my body was used to taking T3 so never had problems doing that but that doesn’t mean I’d would work the same for you. Consequently on testing I have bloods done before I medicate as normal but I don’t have my bloods done 8 hours after taking my NDT as if it is slow release then it wouldn’t matter and I never get symptoms suggesting I’m doing it wrong. Don’t go over doing it though until you are settled on your dose as you don’t want to be depleting your reserves as you titrate up.
