Hello ...I've done my blood test and my TSH is 0.34 ..so my doctor advice me with the Same dose as before (I. e) Levo of 125 mg..and about my hoarseness voice.. He said that my case was so complicated that's why it needed some more time to recover... Just want some advice from you guys... My passion is running out coz of my voice it's nearly four months... M so worried

In my experience, the TSH is only useful to get an initial diagnosis and then, possibly, to tell if you are on enough T4. I say "possibly" in the second scenario because many, like you, end up with a TSH that some would describe as "optimal", yet remain symptomatic. I belong to that category of patients as well.

What you have to do then is to have your free T3 and free T4 tested. Only that can give you an idea of how well you actually convert the inactive hormone T4 to the active form, T3.

If you have any remaining symptoms of hypothyroidism, the TSH will tell you nothing and you need BOTH your free Ts tested. The free T4 levels on their own won't tell you much either, as a person can have high FT4 levels, yet not convert enough of that to T3 so they remain hypothyroid regardless of their TSH and free T4 readings.

Only your free T3 can tell you how well you convert T4 to T3, thus giving you a clearer picture of how well you respond to levothyroxine mono therapy.

I'm not in the UK, but I understand from other posts here that you may have to order private tests in order to get your free Ts tested, especially your free T3 levels. Members in the UK will be able to help you there.

Most doctors only care about and therefore test the TSH which they believe will give you the whole picture. It won't!!! As I said, the TSH can be used to diagnose you as either hypo or hyper (before you are on any meds) but, after that the TSH on its own is useless and should NOT be used to determine your dose or the best kind of treatment for you!!!

Tsh only tells you how much your thyroid is being stimulated. It doesn't tell you how much thyroid hormone you have in your blood for your body to use. That is ft4. Your body has to then process the ft4 into Ft3. It is the Ft3 that our cells need to make everything work. That is why we need to have ft4 tested and ideally Ft3 tested at the same time.

If your case is so complicated then your doctor should be testing more that tsh. I would make an appointment and ask why that’s all that’s been tested. Good luck
