Hi

i have just got my blood results and an concerned about a few.

My cortisol ( at 2.15 in the afternoon) was 381nmol/L does anybody know what this means and are the ranges (looked up on the internet) 30 - 10?

My Vitamin D was 54nmol/L can anybody tell me the correct ranges in the UK? I think it is normal bit the Phostphate result is low 7 mol/L. Can anybody tell me what this is and is it related to Vitamin D? as the consultant has recommended that I start taking 16000 units per day?

HbA1c is 42nmol/mol which I think is pre -diabetes?

I have raised my weight gain and issues with the Consultant for years and he always dismisses my concerns. I am now on 60mcg Liothyronine per day have no energy and have had depression for years.

He also suggests that my depression is not related? when I know that it is.

He does prescribe me T3 only and he does to the necessary vitamin tests.

I hate going to see him as he never listens to what I am saying about my continuing symptoms.

There are no standard ranges for anything thyroidy, and you can't look them up on the internet. The range depends on the machine the lab uses to analyse your blood, and varies from machine to machine. Also, with cortisol, the range depends on the time of day. So, you have to get the ranges from the lab that analysed your blood, you can't get them from anywhere else. :)

There are categories for Vit D - Deficiency, Insufficient, Sufficient (or Adequate). Each category has it's own range.

The NHS will treat a deficiency if the test result is <25, they will usually only prescribe D3 until the level reaches 50 then patients are usually told to source their own.

The Vit D Council recommends a level of 125nmol/L (50ng/ml) and the Vit D Society recommends a level of 100-150nmol/L (40-60ng/ml).

If you want to improve your current level of 54nmol/L (21.6g.ml) to the recommended level the Vit D Council suggests supplementing with 3,700iu D3 daily

Retest after 3 months.

When you've reached the recommended level then you'll need a maintenance dose to keep it there, which may be 2000iu daily, maybe more or less, maybe less in summer than winter, it's trial and error so it's recommended to retest once or twice a year to keep within the recommended range. You can do this with a private fingerprick blood spot test with an NHS lab which offers this test to the general public:

There are important cofactors needed when taking D3 as recommended by the Vit D Council -

D3 aids absorption of calcium from food and K2-MK7 directs the calcium to bones and teeth where it is needed and away from arteries and soft tissues where it can be deposited and cause problems such as hardening of the arteries, kidney stones, etc.

D3 and K2 are fat soluble so should be taken with the fattiest meal of the day, D3 four hours away from thyroid meds if taking tablets/capsules/softgels, no necessity if using an oral spray

Magnesium helps D3 to work. We need Magnesium so that the body utilises D3, it's required to convert Vit D into it's active form. So it's important we ensure we take magnesium when supplementing with D3.

Magnesium comes in different forms, check to see which would suit you best and as it's calming it's best taken in the evening, four hours away from thyroid meds if taking tablets/capsules, no necessity if using topical forms of magnesium.

16,000 units of what? Or do you mean 1,600 units. That can't possibly be 16,000iu of D3 as that is more than the loading dose when deficiency is present.

According to DiabetesUK:

Normal, Below 42 mmol/mol

Prediabetes, 42 to 47 mmol/mol

I'm afraid I can't help with serum cortisol tests.

Your cortisol level looks completely normal 😀

Cortisol is highest first thing in the morning and would be around 500 nmol at that time, it then reduces through the day, with late afternoon / early evening being the lower end. It continues to decline, before starting to increase again around 4am, in time for us to wake up around 6-7am.

You would only need to be concerned if your cortisol result was less than around 130 nmol.
