I would really appreciate your thoughts on my 14year old granddaughters lab results. She currently takes 25 and 50 mcg levothyroxine on alternate days.
TSH. 3.20 (0.2-4.2)
FREE T4. 16.5 (11.0-23.0).

How does she feel?
Presumably she has Hashimoto's?
On Levothyroxine an adult needs a TSH much lower than this. (Under 2) But obviously it may be different for a child
Is she being seen by a thyroid specialist paediatric endocrinologist? If not perhaps she should be
For full Thyroid evaluation she need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested. Also extremely important to test vitamin D, folate, ferritin and B12
Low vitamin levels are extremely common, especially if Thyroid antibodies are raised
Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and fasting. This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, best not mentioned to GP or phlebotomist)
Last Levothyroxine dose should be 24 hours prior to test, (taking delayed dose immediately after blood draw).
Ask GP to test vitamin levels and discuss referral to endocrinologist
Private tests are available. Thousands on here forced to do this as NHS often refuses to test FT3 or antibodies or all vitamins

Medichecks Thyroid plus ultra vitamin or Blue Horizon Thyroid plus eleven are the most popular choice. DIY finger prick test or option to pay extra for private blood draw. Both companies often have special offers, Medichecks usually have offers on Thursdays, Blue Horizon its more random
Blue Horizon will agree to test minors. Medichecks will not
Best option is to pay for private blood draw rather than DIY finger prick test
