Should I be concerned?

I had a blood test in February 2017 for Vit D of which I know I am deficient. Also at the same time I was tested for Inorganic Phosphate the results of that were 1.19 (0.8-1.5) the Vit D was 43. Was given Vit D 40,000 a month for 3 months. TSH 0.27 within ranges.

Tested again June 2017 - Vit D 92, Phosphate was 1.37. TSH 1.1

Tested again Aug 2017 - Vit D 70 not tested for Phosphate. TSH 0.27

Tested for TSH Mch 2018- 1.4

Tested again June 2018- Vit D 68, Phosphate was 1.49. TSH 0.54

Should I be concerned about the Phosphate levels? I don’t feel too good, slight pain in kidney area, stomach on overdrive and aching joints. TSH is a bit up and down.

TSH range (0.2-4.0)

Vit D range (0.75-250)

I think you should ask your doctor what is causing high phosphate levels. It needs investigating if it remains high. Have you checked bone density?

I think you should also be concerned about your Vit D level.

It was fine to give you 40,000iu per month for 3 months but that only worked out at approx 1400iu daily. That's not really enough to reach the level recommended by the Vit D Council, the Vit D Society and Grassroots Health which is 100-150nmol/L

From the way your Vit D has steadily reduced, I assume you took the 3 months' worth of Vit D supplement and then nothing. Once the level has been raised, we need to continue with a maintenance dose.

Your June 2018 level of 68nmol/L, according to the Vit D Council, requires the following to reach the recommended level:

To reach 100nmol - 2000 IU daily

To reach 125nmol - 3700 IU

To reach 150nmol - 5800 IU

As we'e now approaching winter and can't make Vit D naturally from the sun, we draw on the Vit D we make and store during the summer, so your level will continue to reduce unless you supplement.

Your doctor wont know, because they are not taught nutrition, but there are important cofactors needed when taking D3 as recommended by the Vit D Council -

D3 aids absorption of calcium from food and K2-MK7 directs the calcium to bones and teeth where it is needed and away from arteries and soft tissues where it can be deposited and cause problems such as hardening of the arteries, kidney stones, etc.

D3 and K2 are fat soluble so should be taken with the fattiest meal of the day, D3 four hours away from thyroid meds.

Magnesium helps D3 to work. We need Magnesium so that the body utilises D3, it's required to convert Vit D into it's active form. So it's important we ensure we take magnesium when supplementing with D3.

Magnesium comes in different forms, check to see which would suit you best and as it's calming it's best taken in the evening, four hours away from thyroid meds

As for your fluctuating TSH levels, are you always doing your test the same way - ie

* Earliest appointment of the morning for blood draw - no late than 9am

* Fast overnight which just means delaying breakfast until after the blood draw. Water is allowed but no coffee, tea, milk, etc, until after blood draw.

* Leave off Levo for 24 hours.

Ensuring conditions are the same every time means you can accurately compare results. TSH is highest early morning and lowers throughout the day, so your TSH at 8am will be higher than at 10am for example.

Also, TSH is not the whole picture. You need FT4 and FT3. If your GP doesn't do these tests it would be a good idea to do what hundreds of other members do and that's a private test with one of ThyroidUK's recommended labs. Now I have finished the Vit D course I have to wait 6 weeks then another blood test. After the blood test I am being prescribed Calci D 1000 a day.

Yes I do make sure I am tested for thyroid as you state. G.P won’t test anything unfortunately. At the moment I can’t afford private tests but have done previously.

What doseages for Magnesium and K2 should I take please?

I also feel so cold and tired!

I'm surprised your GP is retesting Vit D, so many of them wont due to cost.

After the blood test I am being prescribed Calci D 1000 a day.

That contains the equivalent of 1000mg calcium and 1000iu D3. Has calcium been tested and are you deficient so that you need to supplement? If not tested then I would ask your GP why he is prescribing it and ask him to test first. Taking D3 increases the amount of calcium, as explained above. And how does your GP know that 1000iu D3 will be enough? It can be a maintenance dose for some people when they reach the recommended level of 100-150nmol/L. Personally, even though my level is now within the recommended range, I need 5000iu daily throughout the winter and at least 2000iu daily in the summer, in fact I've been taking 5000iu daily for the last 3 months because I was getting bone pain with a Vit D level of 104.

What doseages for Magnesium and K2 should I take please?

Whatever form of magnesium you decide is best, you just take what it says on the pack. With K2-MK7, 90-100mcg is sufficient for up to 10,000iu D3.

I too thought that about calcium.

My results were Serum adjusted calcium concentration 2.34 (2.2-2.6) . I wondered if she is prescribing these because of my age being 63?

Personally, I would be very wary of taking Calci D.

My calcium results

2002

Calcium: 2.36 (2.2-2.7)

May 2018

Calcium: 2.3mmol/L

Calcium adjusted: 2.39mmol/L (2.2-2.6)

and mine has always been considered good.

Age shouldn't come into it, I am 70 and there was no mention of giving me calcium after my May result which is very similar to yours.

If you're GP is thinking about osteoporosis, then ask for a DEXA scan. Check out Dr Myhill's articles

The first consideration should be to get your Vit D level up to that recommended by the Vit D Council and others.
