New on Nature throid and tsh 29

Hello,

6 weeks ago I was on Levo 125mcg and my levels were 1.75 tsh, 1.5 ft4, and ft3 2.6. This was in the morning before medicine

Doc switched me to nature throid 1.25 grain and now tsh is 29, ft4 .87, and ft3 2.4.

I thought 1.25 grain was equivalent to 125mcg Levo?

Now doc wants me on somewhere between 1.75-2.25 . At first she said 2.25 but I was worried about the t3 being too much .

What do you think?

Also, I rechecked it taking my medicine 2 hrs before labs just to see the difference and it was 19 tsh ,ft3 3.5 And ft4 .86. So clearly it’s the ft3 bringing the tsh down.

My concern is that I get pregnant easily (I have had 5 losses and no live births) and we just started to try to conceive just before finding out these awful tsh results. For pregnancy they want it under 2.

Does anyone also know which thyroid hormone baby relies on in pregnancy ? Is it tsh, ft4,ft3? In other words, is there any hope for a baby if I happen to be pregnant now?

And how quickly can I see a change in ft4 in labs ?

I've had the condition since birth. However I from talking to people who develop this condition later in life its a process of trial and error. To get your levels right. I'm not sure what the levels should be off the top of my head.

My friend in her twenties is having the same issues.

As to your other question pregnancy is tricky. And I only wish I could promise you your hearts desire.

I'm not even sure if my answer has helped but I couldn't not reply based on your question.

So sorry you have lost your babies. I feel for you but I would stop trying for the moment to get things right and stablized. Then your much wanted baby will get the best chance of going full term. Keep posting your bloods and we can comment on your progress and encourage you.

Grains and Levo don't correlate that easily I'm afraid. I take grains but I think it's roughly one grain equivalent to 38 T4 and 8 T3. I haven't got access to my dose at the moment so can't say that's accurate and there may be slight variation between brands. To give you an idea I now take 1.75 grains and I was taking 120/125 Levo but as I say that's just a rough guide. It should say how much T4 and T3 is in it in the packaging. Hopefully if you cant find it then someone taking the same can help you. Have you just stopped T4 and started on grains? I think I stopped and then followed the STTM ( Stop the Thyroid madness book) way of doing it but still took too much as that recommended starting with half a grain and adding in another half up to two and then only increase in quarters. So going up to 2 I missed the 1.75! So a lot is on how you feel but it is essential that you test frequently and that must include testing T3 as that is the most important reading when taking any form of T3. Some hospitals will insist that you don't take any form of T3 went pregnant so that is something else to think about. So do you have any readings whilst just on Levo and why did you start on NDT? I'm not trying to burst your bubble but just thinking of what obstacles you may have ahead of you and seeing if there are alternatives

Thanks for all this !! I got a positive pregnancy test Sunday at 3 weeks 3 days! And lines have gotten darker !! I decided to take the 2.25 grains and my levels yesterday came back as :

Tsh 6.4!!! Yay!!

Ft4 2.6

Ft3 2.7

Hcg was 35 (average for 3 weeks is 5-72)

Progesterone 44 (range goes to 45 so that’s good)

So I am thinking I will follow docs advice and continue this and see where numbers fall next week :)

You’re still vastly undermedicated with such results. Aren’t you testing a bit too frequently too? Usually we have to wait 6 weeks to test for new dose or new meds (and every dose increase in that)

Congratulations on your happy news.

To sustain a pregnancy, you need to keep your TSH under 2 (preferably lower) and you will most likely need a 30-50% increase in your meds as early as the 4th week of pregnancy. If your TSH is 6 now, you may not be on the correct NDT dose even without the additional requirements of the pregnancy. You probably need another increase right away, although it looks like you haven't been on your current dosage for long enough for your recent test to mean anything. It's difficult to titrate the dose properly if you weren't stable on your meds pre-pregnancy, so be aware of that.

When you have a test, you must have the test as early as possible in the morning before eating and most importantly, before taking your meds.

Can't really advise further than that because you haven't stated your lab's ranges for FT4 and FT3.

Many doctors aren't up to speed on requirements during pregnancy, so it's wise to do your own research. The situation seems even more complex when on NDT because those doctors that do understand thyroid requirements during pregnancy, seem only to know how to dose Levo, and not NDT during pregnancy, so that coupled with the recent change in meds is going to make it challenging to get things right in your case.
