You need Levothyroxine replacement because your FT4 is below range.
Low-normal TSH rules out primary hypothyroidism but FT4 below range indicates secondary or tertiary (central) hypothyroidism. Central hypothyroidism means your thyroid gland is not getting sufficient stimulus to produce T4 and T3 hormones. Treatment is Levothyroxine to replace low thyroid hormone but it is usually managed in endocrinology as investigation should be made into possible deficient sex and growth hormones too.
Show your GP the NICE CKS which says:
Secondary or central hypothyroidism is the result of insufficient production of bioactive TSH due to a pituitary or hypothalamic disorder.
You need further investigation
Low FT4 with low TSH is unusual
Normally with low FT4, TSH would be above range
Looking at central hypothyroid as possible cause
Ask for TPO and TG thyroid antibodies to be tested
Also vitamin D, folate, ferritin and B12
Have you had whiplash or head injury?
Take other medication

Hi Clutter and Slow Dragon thank you both. These are husbands results (I’m a hypo and wondered if he was given his symptoms).
Yes neck fracture 2004... also POTS since 2014 mand so far endo has said sick euthyroid (T4 was 12 ish)... has had adrenal tests in past because of POTS and CFS. We wonder if low blood pressure drugs have caused problem with adrenals. Clearly T4 low though looks like conversion may be ok. Vitamins etc all been ok in past tests as were antibodies (negative).
This week had angina type attack and now high blood pressure. Trying to unravel it all.
