First of all your TSH for diagnosing was over-range and he should have begun with a 50mcg dose and every six weeks thereafter a blood test and increase of 25mcg until TSH is 1 or lower with FT4 and FT3 in the upper part of the range.
As you can see with your dire results, both Frees are below range.
You must feel awful and I'll give you a list of symptoms. Tick them off and take them when you have your next consultation.

You must also ask for B12, Vit D, iron, ferritin and folate. Everything has to be optimal - not 'normal' as most doctors believe.
The aim is to be symptom-free. Levothyroxine is T4 only. T4 has to convert to T3. T3 is the active thyroid hormone required in all of our millions of receptor cells as it is T3 which alleviates our symptoms.
All tests for thyroid hormones have to be at the very earliest, fasting (you can drink water) and allow a gap of 24 hours between your last dose and the test and take it afterwards. If we take levo or any thyroid hormones before a blood test the results will be skewed.
Tell your doctor you a member of NHS Choices for help/advice Thyroiduk.org.uk and tell him you have to have 25mcg increase every six weeks until TSH is 1 or below and FT4 and FT3 in the upper part of the range.
Many doctors unsurprisingly believe that once the TSH is somewhere in range (in your case between 0.2 and 4.2) that you are on sufficient.
Levo should be taken with one full glass of water, usually when awakening and wait an hour before eating. Food interferes with the uptake.
As you start to improve don't use up your extra energy as you need to build things up a bit. You must be feeling dreadful at the moment. Getting your vitamins sorted will help a lot as well. A lot of us have to supplement for that as we are often low.
