Dose advice please?

Hello! So I have Hashimoto’s hypothyroidism, the DIO2 gene mutation that prevents T4 to T3 conversion, Type 1 diabetes, a history of iron/vitamin d deficiency, sleeping problems (delayed circadian rhythm), and anxiety/depression. So it can be very hard to determine why I am feeling tired or not myself. My recent blood test:

TSH 0.04

T4 12.8

T3: 3.7

Vitamin D: 66

I am currently on 150mcg of Thyroxine and 20mcg of T3 (divided into twice a day) and take everyday multivitamins. I have forgotten to take my thyroxine a few times but I don’t believe it’s been that many times to make a huge difference. My last blood test showed TSH 0.01, T4 17.8, T3 4.5.

My question is should I consider changing my dose, or changing the dose timing (I have just discovered that thyroxine shouldn’t be taken with iron), or whatever else. I’m not really sure where to begin. Thank you!

Neither thyroxine nor T3 should be taken with anything else. The rule is :

- on an empty stomach, one hour before or two hours after eating or drinking anything other than water

- two hours away from any other supplements or medication

- four hours away from iron, calcium, vit D3 or oestrogen - and I find it best, personally, to leave six hours between thyroid hormone and magnesium.

Multivitamins are a total waste of money, for many reasons. You'd be far better off getting vit D, vit B12, folate and ferritin tested, and just taking what you need, in quantities your need, according to the results.

Don't change anything in your dose until you have been taking your hormone correctly for at least six weeks, and then test again. :)

Get yourself a weekly pill dispenser for your Levothyroxine. Keep it by the bed. Essential to take EVERYDAY

Always take Levo on empty stomach and then nothing apart from water for at least an hour after. Many take on waking, but it may be more convenient and possibly more effective taken at bedtime

Many people find Levothyroxine brands are not interchangeable. Once you find a brand that suits you, best to make sure to only get that one at each prescription.

What are most recent results for folate B12 and ferritin including ranges

Vitamin D is too low (assuming you are in UK)

Aim for around 100nmol. What dose are you currently taking? Do you also take magnesium and vitamin K2?
