Newbie: Not felt right for Decades. CFS or hypo thyroid or both? Advice needed on action please!

Hello! I am desperate for answers so hope you won't mind if I describe what I consider significant aspects of my medical history as I am not sure what is relevant and what is not... it may take a while, but here goes.

I have been struggling with energy levels on and off for many years with the episodes apparently getting closer together and taking more of a toll. (Of course, I am getting older too!)

I think it all started in 1984 when I had glandular fever. Afterwards, i seemed to tire very quickly and my glands were often painful. In 1988 my then GP said she thought I may have 'post viral syndrome'. Untreatable, of course.

In 1993 I had pre-eclampsia when pregnant with my second child and he was born 2 months premature. I moved and changed jobs in 1995. In 1997 I went to my doctor (still my current GP) feeling exhausted and he ordered a thyroid test. I remember him saying I had 'immunoglobulin g' but not much else, except that I had enough thyroxine for now, but as my body was killing off my thyroid I would need medication in the future. In 1999 after I had had my third baby, I was started on Levothyroxine. Over the next few years I went up by degrees to 200mcg, until I had an SVT event, and then dropped back down gradually to 125mcg which has remained my daily dose.

Although I was being medicated, I complained to my GP of feeling fatigued and was sent to a specialist (cannot remember what he specialised in!) and was diagnosed with CFS. I continued to work full time, but seemed to have a lot of episodes of flu -like symptoms which could have been CFS flare-ups, actual viruses, or, now I am wondering, were they thyroid-related? It all came to a head 4 years ago when I ended up being 'blue-lighted' to hospital feeling very unwell indeed with irregular and rapid HR and sweating. A heart attack was diagnosed after the ECG showed up inverted t-waves, but that was overturned when a blood test (I think it was for an enzyme that is released post heart attack) came back negative. I was exhausted, though, and after 3 months off work, tried to go back twice; however I could not get beyond 50% in the 6 week time-frame given each time. My husband and I decided the best thing was for me to stop and get better in my own time, so I resigned in 2014. I thought I would get back to health and work within a year, but it has not happened. I cannot progress on graded exercise programmes. I am going round in circles trying to sort myself out! At the moment I am feeling unwell with sore throat, headache, glands up in neck and arm pits and aching limbs. I get sinusitis a lot. Even when I am not feeling unwell like this, I have good days when , for example, I can walk up the (fairly gentle) hill to the post box, and other days I have to stop to take a breath, or get puffed out going up the stairs. I cannot find a pattern as to when I might feel better or worse. I keep wondering, is it CFS? A virus? My thyroid? I have very little stress now as I am not working so it seems unlikely to be psychological. I have asked my GP but as my TSH and T4 are normal, my GP says to soldier on. I have been prescribed iron in the past (I am pescatarian and have heavy periods) so I will post the historic ferritin levels I have:

Sept 14 : 14ug low value, so 6 months prescribed iron

Mar 15: 34ug told this was ok. Iron stopped.

July 16: 31ug further 6 months iron prescribed.

Aug 17: 33ug. Doctor maintains iron prescription unnecessary. I am taking OTC iron (14mg daily)

My most recent TSH is 0.08mu/L and T4 18.5 pmo/L.

I am thinking of getting some blood tests done privately so I can at least know whether or not my thyroid is causing me these problems as there is no test for CFS.

Any thoughts and advice greatly appreciated. Many thanks for reading.

TSH 0.08 with FT4 18.5 means you are unlikely to be undermedicated on 125mcg but it doesn't show whether your FT3 is good or bad and it is low FT3 which makes you feel hypothyroid. If you can order a private thyroid test to include TSH, FT4 and FT3 we will be able to see whether or not your FT3 is good or whether the addition of some Liothyronine (T3) might be beneficial.

Ferritin is usually optimal around 75 - 100. I would take Ferrous Fumarate 210mg which has 65mg of elemental iron. Take it with 1,000mg vitamin C to aid absorption and minimise constipation. Take iron 4 hours away from Levothyroxine.
