Hi I have spoken to practice manager today about how badly I've been treated and she has given me an appointment tomorrow morning to discuss all my concerns! Hopefully get my medication increased from 50mcg levothyroxine and have my vit D reinstated as well as further investigations? Wish me luck!

You definitely have Hashimoto's Thyroiditis. All of these blood results show that you are undermedicated for thyroid and both of your antibodies are very high which show Hashimotos instead of Hypothyroid. You must be feeling very ill.
A bit more info to arm you ready for the Practice Manager. Your TSH should be under 1 to feel well (the lower the better), your Free T4 is in the lowest part of the range, Free T3 was not tested and it's the most important blood test for Hypo/Hashi's, vitamins D and B12 are very low and need boosting, folate and ferritin are off the scale downwards. Marigold22 has summed it all up. All your blood results are very low. The range for ferritin in itself is low, which shows that there is a lot of difference between labs. My levothyroxine has been gradually increased from 25 mcg to 75 mcg, over 3 years and I am still struggling with symptoms. Antibodies were raised originally, but have not been since or not been tested. I am discussing my recent blood results with a new GP in the practice next week...
