I can't see any swelling/goitre it looks normal to me.
Do you have TSH and FT3 results in addition to FT4?
free t3 4.85 range 3.1 - 6.8, TSH 1.92 range 0.27 -4.2 FT4 23.4 range 12.0 -22.0

TSH and FT3 are euthyroid (normal) so despite high FT4 you are not hyperthyroid.
Have thyroid antibodies been tested? Having autoimmune thyroiditis may cause high FT4 or high FT3 but will usually push down TSH too.

Autoimmune thyroiditis (Hashimoto's) causes 90% of hypothyroidism. There is no treatment for it, treatment is for the low thyroid levels it eventually causes.
Ask your GP to do a thyroid peroxidase antibody test to confirm or rule it out.
