You have to phone your doctor immediately and get a prescription for levothyroxine. It should be 50mcg and every six weeks thereafter an increase of 25mcg of levo with a blood test.
I'd ask to be referred to an Endocrinologist if you are in the UK.
Levothyroxine should be taken first thing on an empty stomach with one full glass of water and wait about an hour before eating.
You have to be feeling absolutely horrendous and I don't think you'll have the energy to go to the doctor so phone and ask for phone consultation as your TSH is extremely high. The pharmacist should be able to deliver it, I believe if GP sends prescription electronically.

On re-thinking my answer, I think you should phone your GP and ask him to contact an Endocrinologist to see if a higher dose than 50mcg would be an appropriate starting dose.

Your doctor should prescribe Levothyroxine. If you are under 50 years of age and don't have heart disease 75-100mcg is an appropriate starting dose.
The good advice is good. I would see your GP or family doctor and ask for another blood test as the one you have is unusual. They normally measure 'free' thyroid hormones using immunoassays. I'm not sure how good chemiluminescence is and total T3, total T4 are not much use. So it would be good to get a more useful blood test and a prescription that you can start taking as soon as the blood has been taken.
(The comments on the report are a bit amateurish, which makes me think another blood test by a doctor would be useful as confirmation of your precise levels).
To be sure you should be retested in different laboratory and different method. Unfortunately for example dialysis method is expensive and takes long time to get results so many doctors ignore it.

I would start with 1 x 100mcg tablet and have a thyroid function test in 6 weeks to check thyroid levels. If FT4 is still low and TSH is over 1.0 you could increase dose to 125mcg.
For maximum absorption Levothyroxine should be taken with water 1 hour before, or 2 hours after, food and drink, 2 hours away from other medication and supplements, and 4 hours away from calcium, iron, vitamin D supplements, magnesium and oestrogen.
It takes 7-10 days for Levothyroxine to be absorbed before it starts working and it will take up to six weeks to feel the full impact of the dose. Symptoms may lag behind good biochemistry by several months.
You should have a follow up thyroid test 6-8 weeks after starting Levothyroxine. Arrange an early morning and fasting (water only) blood draw when TSH is highest, and take Levothyroxine after your blood draw.
