DWSmith, the T3 element in NDT suppresses TSH and often lowers FT4 as the oral ingestion bypasses the need for high levels of FT4 for conversion.
You look to be optimally medicated so have your ferritin, vitamin D, B12 and folate checked before making any adjustment to your NDT. Low/deficient levels of ferritin, vitD, B12 and folate can cause musculoskeletal pain and aches, fatigue and low mood.
The only way to raise T4 would be to take extra T4 but this will probably overmedicate you unless you reduced your NDT too.
TSH means very little, when you are taking any form of thyroxine it can become suppressed. T3 needs to be at the top of the range and T4 is a storage hormone only. I would increase T3 before T4 if I were you!
