unending anal pressure

hello, looking for ideas to solve this ongoing problem. I appreciate your help and feedback in advance.

I (24M) have never really had any problems with my digestive system my whole life, but this issue of constant anal pressure has slowly progressed over the past few months. It feels like I need to pass gas or stool to make relieve, but there’s nothing. Also kind of feels like my anal tissue is flared out , or not closed fully, kind of like immediately after a bowel movement, but that feeling just remains constantly. Passing gas relieves feeling somewhat.

When I wake up, the feeling is there a bit, but almost always feels normal after going to the bathroom in the morning to pass gas. After taking a shower and getting ready, I feel normal for several hours. Usually have a strong urge to pass stool around 1pm. Afterwards, I usually feel decent, and then the pressure creeps up and stays there most of the day.

I started noticing this issue getting much worse and happening almost daily after a couple days of constipation several weeks ago where it felt like I didn’t evacuate all the stool and I was straining a lot. After that time, I would sometimes strain somewhat on the toilet, and it almost feels like something is coming out but it’s nothing. Almost feels like my anal tissue pushing out and going back in. Whenever this happens, the uncomfortableness gets way worse for the remainder of the day. As a result I try to avoid straining and this reduces the discomfort somewhat, but does not eliminate.

I have tried several treatments such as hemmeroid ointments, tuck cooling pads, anusol suppolisites, and body powder. I also take 2 capsules of Seed every day which is a probiotic. These all have helped somewhat but do not have lasting effects. They maybe work for a couple hours before symptoms return. Using gold bond body powder has the most success in terms of eliminating the symptom, but it usually only lasts an hour or two.

I honestly don’t understand why this issue is so persistent. Nothing really hurts, nothing is strange or feels weird during bowel movements, no blood, no stomach pain. It doesn’t fully match the symptoms for hemorrhoids, anal fissures, or prolapses. If I had to guess I would say some kind of long lasting hemorrhoids because it almost feels like inflation inside that gets worse when straining.

I would greatly appreciate any recommendations for strong numbing agents or other treatments that might last 24 hours… thank you.

I have similar systems. I think for me it’s part of hsving IBS. It’s very annoying. I don’t think doctors understand it. If it were me, I would my used talcum powder since it has been associated with Cancer in women.

you need to see a gastroenterologist and get a Manometry and Defecography

I've had similar symptoms for years and IIt turned out to be intrarectal intussusception. Only a defecography el detect it. If you have many years of IBS-D your rectum will suffer. Need pelvic floor therapy or maybe even surgery. There are few answers for these problems unfortunately and they're very bothersome. You need to add fiber to your diet soluble and insoluble to make your stools solid so that they pass easily . If it progresses it may come out. Go get tested by you gastroenterologist.

This could also be a slight rectal prolapse as you have been straining with constipation. I have had similar after the births of my children years ago and was diagnosed with this and straining can aggravate it. It can make you feel as if there is something coming down or stool to be passed when there isn't. It could also be connected to haemorrhoids, but I would suggest you see your GP to have an examination in the first instance.

Go get a rectal exam. Eat porridge or incorporate some kind of oatmeal into your diet. This is the only thing that helped me. It also helped what felt like a hemmorhoid. The only ointment that real helped in years gone by is Proctocedyl which is a prescription. All the best to you.
