Had a Radical Prostatectomy with clear margins, and for a year have had

Vacation time again
My
PSA
is down a notch to 0.2. We decided to follow the original plan of intermittent ADT and I’m starting another ADT vacation. This round of ADT had started with minimal SE’s but in the last month or so it’s gotten worse.

Worsening Stress Incontinence 20 Months After Surgery?
My

is 0.02. My best guess is that physical shape has a direct impact on incontinence post-surgery and it mainly because I'm not as in as good of shape as I was when my continence was better?

Asking employer for accommodations - yes, no, maybe?
Started Firmagon in May 2022 after nothing seen on PSMA PET/CT with

at 0.4. Switched to Orgovyx in November since injection site reaction was compromising QoL. Just before BCR was detected, I started taking adderall for ADHD-inattentive. My issue: I'm a 62 year old Electrical Engineer.

Update on Prostate Cancer Vaccines that I follow... another year is gone...

Back to doxetol
After seven months on cab it kicked my ass, two months now back to doxetol that I had in 2018.my
PSA
rise to 502.4, after the first infusion it dropped to 194.4 a much needed surprise. I am scheduled for a consultation with radiology to see what there plan is for the lesions on the liver.

Pluvicto Update: Second Treatment Coming up
Unfortunately, my
PSA
has not dropped a whole lot. It went from 248 to 222. My Alk Phos shot up from the mid 300s to almost 600 now. Hoping things improve after the next infusion. Hope it's just dying cancer cells affecting the
PSA
.

was 28. I was given a 3 month shot of lupron in June 2022. In August I had 20 sessions of IGRT - Image guided radiation therapy. These are

Are there a nomograms or similar tools that would predict recurrence after IMRT/IGRT (without ADT).
In 2015, I underwent 45 sessions of radiation (no ADT) for a small amount of 3+3 and even smaller amout of 3+4, with a pre-radiation

of 1.7. My

has stabilized at .2 for the past two years. Can't find any post radiation predictive models or tools.

Treatment Decision: Now considering surgery
As a reminder I am Gleason 8/9 (all 12 cores) with a

that has been hovering around 25 since October. I am paraphrasing Dr. Brachy but he essentially said, "I rarely recommend surgery but it does happen.

When to start ADT, Salvage radiation or both for BCR after PSMA Petscan shows nothing?
The PSMA scan came back with no indication of the
PSA
source, so now I am wondering about next steps. (At least I have a baseline scan now.) Obviously, I will continue to monitor
PSA
for doubling time.

Status at PSA nadir
Here is my latest update of the
PSA
curve. Note as before that
PSA
is plotted on a log scale to enable the small changes near nadir. The most recent result is 0.1 - which is the same as 3 months ago. What does this mean?? I have made 3 suggestions for how to interpret this: 1.
