The battle is over
I will miss him alot In the last month, he became increasingly weak after the second chemo (
docetaxel
) treatment, so treatments were abandoned. Metasisis had moved into the shoulders and a spot was seen at he back of the cerebellum just above the spine.

Novartis Pluvicto financial aid? Company insurance so-so self insured crud. Also, do enlarged pelvic lymph nodes still qualify you for it?
Age then, 54 (Born 1961) now going on 62 with still slow progression, but not liking
docetaxel
side effects. Want A vacation with Pluvicto before starting Cazabitaxel, hope I can qualify.

Cabazitaxel update and variations of PSA
Hi all, those who have followed my husband's history will know that
Docetaxel
did not work for him although the MO insisted on 4 infusions. Cabazitaxel was given on 10 Sept (nearest PSA was 468 on 2 Sept in London).

Tailbone Spasms from Neulasta
After my second dose of chemo (
Docetaxel
), I ended up in the ER with a fever. My white blood cell count was 1.2 K/CMM and Neutrophils were 0 K/CMM (!). Anyway, I was approved for Neulasta and had the injection 24 hours after my 3rd chemo infusion.

Cabazitaxel first infusion on Saturday
For those who have been following my husband's progress,
Docetaxel
has not worked for him at all. The NHS pushed for 4 infusions although we were requesting to change to Cabazitaxel after two. PSA rose from 99 pre-chemo to 468 now and LDH from 342 to 1373. ALP modestly increased to 130.

Monitoring Lactate dehydrogenase (LDH)
In June, before starting
Docetaxel
, his PSA was 99 and LDH 342.
Docetaxel
4 is due today - his PSA is 414 and LDH 1250. If you look at papers on LDH, it seems to have a role in PCa development. If something does not affect the treatment choice, it is not monitored.

Need help finding studies
And then the doc’s post-visit notes recommended Zytiga or to consider treatment with
Docetaxel
, or a bunch of other meds… all the stuff we had asked about but he had said wasn’t appropriate when we were in the office with him. (Argh.)

What next?
After 4
Docetaxel
and 2 Cabazitaxel infusions, PSA went up from 99 in mid June to 821(676) last measured. On Monday night this week, my husband could not walk to go to the bathroom and was in immense pain in his left hip and leg.

not sure my Onco (or I)understood the Denmeade BAT paper.
Onco decided to delay Xtandi and go to
Docetaxel
to bring down PSA I had 6
Docetaxel
treatments. The last two were accompanied by Carboplatin and 40mg and 80mg of Xtandi respectively. This lowered PSA to 6. That was three months ago.

Pluvicto / Locametz in the Rocky Mountains yet?
Chemo with
Docetaxel
since Aug 2021, possibly starting to fail. Wanting the new Locametz / Pluvicto treatment before starting Cazabitaxel, but availability in the SLC / DEN area non-existant yet. Huntsman Cancer institute hopes to have it sometime in this fall, but no date given.

Is Pluvicto worth pursuing?
Dropped Xtandi and completed 6
docetaxel
sessions ending August 2022. Recent PSMA PET scan showed most lesions decreased in size/intensity with two sites (rib and hip) showing further progression.

Sequence of treatments
Given my husband's post Enzalutamide
Docetaxel
treatment and the lack of a great response (so far), I had a look around for sequencing to see if
Docetaxel
might have been more effective before giving Enzalutamide.

He's going to start
docetaxel
, again, after having already done it in 2020. Has anyone here taken

Genetic counseling & testing
Just completed six rounds of
docetaxel
+carboplatin last week and will be seeing my doctor at Mayo on March 30 for further scans to gauge progress and evaluate next steps. Meanwhile, my oncologist at MN Oncology has recommended that I do genetic counseling and further genetic and tumor testing.

Guardant Liquid Biopsy - when is best to do this?
On Lupron and 1/2 way thru 6
docetaxel
chemo treatments. Had genomic testing of biopsy sample which showed MMR deficient and MSI high. MO suggested doing liquid biopsy, though unclear on timing.

No benefit of adding docetaxel to ADT+RT in treating localized high-risk or pelvic lymph node only (N1) newly-diagnosed patients
I updated this with the most recent STAMPEDE trial of adding
docetaxel
to ADT in what they call "high-risk" patients. https://www.prostatecancer.news/2016/08/
