Local relapse after HDR Brachytherapy and EBRT
He started ADT (Firmagon) Jan 2019 then had HDR
Brachytherapy
and 22 EBRT. He stayed on ADT for a total of 20 months, his T was not going below 0.7 so he switched to Eligard for the last 6 months.

His suggested course of treatment is 5 weeks of IMRT followed by
brachytherapy
. Lupron would start two months before radiation and last a total of 24 months. No scans yet. PSMA PET scan would be possible, but it would mean waiting 3 to 4 months because of backlog at UCSF.

Update
Otherwise , I wouldn't have ended with metastatic cancer starting from
brachytherapy
for a Gleason 3 + 3 cancer ten years ago. I also had chest Xray, CT scan, and bone scan showing no change from last year.

Bone scans can lie!
When my radiation treatment was planned last year at UCSF, they had left out
Brachytherapy
, Tall Allen suggested I talk to my OC about adding that, the doc said it was a great idea and it got added to my treatment. However good UCSF is, they still missed it and TA caught their mistake.

Plan after PSMA Scan
Everyone said no
brachytherapy
. All the evidence we have point out that external beam is the way to go when a patient has disease in the bone. While we are not 100% certain but the radiologist thinks it is most likely cancer.

Bowel issues
Following stage 1b2 diagnosis I finished surgery, chemorads and
brachytherapy
in September last year. As we come out of lockdown and the summer arrives I’m feeling self-conscious about gas, gel discharge and involuntary bowel movements.

NCCN guidelines for unfavorable intermediate risk
I have Gleason 7 (4+3) unfavorable intermediate risk PCa and I chose the HDR
Brachytherapy
(monotherapy) path. It looks like EBRT & HDR

They have ruled out RP and
Brachytherapy
due to my previous 8 weeks (40 treatments) of Targeted Beam Radiation in 2013. I am in a new position where I don't feel in control anymore, but are pretty happy with the result of the scan.

Update #4 in my continuing journey. Decision made.
One of the worlds leading
brachytherapy
specialists ruled that out for me. I then looked at SBRT / Cyberknife. Met with 3 Radoncs who all said the same thing, that they would not recommend hyper fractionated RT due to my prostate size and history of BPH.

So..my UroOnc wants to pursue cryoablation after my failed radiotherapy (EBRT/LDR

). I don't know yet if I can have focal only cryo or have to have whole prostate ablation. I know the ED outcome is bad, but has anyone else had this treatment and how are you doing afterward?

Plan is for

at UCSF then 25 External beam. Had a PSMA scan Monday with a PSA 0.8 Radiation Oncologist had this to say: I am out on vacation but I have looked at the report.
