best treatment after Xeloda?

I was diagnosed with MBC in Feb 2022 with bone mets. I am HER2 low ER positive.

I have been on kisquali, Ibrance, Faslodex, and lastly 4 rounds of Xeloda. Nothing seems to stop my progression,

Kisquali sent my liver enzymes sky high.

Ibrance/Faslodex progression was slow and markers rose.

Xeloda markers jumped and also progression into my liver and other bones

My oncologist is changing me to IV chemo (my first experience)

The choices are Enhertu, Taxol, or Erbulin.

Has anyone else a similar experience and what treatment was decided on?

Can you try gemcitabine first? Iv chemo. No hair loss. Very effective. That was my move after xeloda

I’ve read that Enhertu is supposed to be very effective. This is a presentation by an oncologist from Dana Farber and she goes through their current thinking about how to sequence drugs. You might find it helpful.

Wonderful! Thank you so much for posting. I'll need to watch it more than once but the data and hope it gives are priceless.

Very helpful video. Thank you so much. It confirms what my oncologist told me that people who get a good run on the inhibitors ibrance etc. tend to do well on the next treatment.

Unfortunately, I am not in that category but Enhertu looks promising.

also of great interest is Trodelvy as mentioned in the video.

The results of the phase 3 Trial TROPiCS-02 show its benefits for HER2 - .MBC

I’m not able to access the link - can you repost it or tell me the full title of the video so I can search for it on YouTube?

It’s not letting me post it for some reason but it should be easy to find; it is called “2023 updates in metastatic breast cancer treatment”

It is slightly outdated - for example Elacestrant is already approved. But it’s still extremely interesting I think.

The other interesting thing is that she (her patients) had a bad time with Piqray. I happen to do ok with it .

Hello Breege

I see this post was 11 days ago -have you made a decision?

After oral chemos stopped working for me -I wasn’t given a choice really, paclitaxel (taxol) was my next treatment. My first IV treatment ever! Had first a scan of Friday, so I won’t really know what’s what until I speak to onc so can’t tell you if it’s working but treatment itself is ok - it’s just long because I’m choosing to cold cap to try and keep my hair aka my sanity!

I also find it confusing when certain drugs are given and in what order/protocol. Had Taxol in 2016. Then anastrozol, Ibrance w/fasoldex end of 2019, then xeloda, venorelbine and lastly, enhertu all in the last year, because none of them worked. Last cycle September. Waiting for scan tomorrow, then same day with doc. I guess it is eribulin next, then taxol. BUT, I want to be tested because I strongly believe my dx may have changed. Being er/pt+ and her2low, enhertu was suppose to be the game changer. After 4 cycles and tm's rising 600+ each month, that too failed. I just hope I am still fixable. :-)

Enhertu is not available in Ireland yet to public patients.

I have private health insurance and it was decided if they agreed to fund it I would try it first. I am due my first infusion next Tuesday. I have been warned that it is not an easy drug to tolerate but then what cancer drug is?

Truthfully, I am terrified. But as they say over here “I will give it a lash” 🤞

Thank you. My appointment was for 7.30 am GMT, due to head home now. Feel very tired but thankful it went ok, I have prescriptions for 4 different anti nausea meds. All to be taken for first few days at least. Obviously nausea is a big side effect.
