hi popcorn, each person is different, so where the pain manifests may be in the buttocks and genitals or in one or the other. It can be referred pain or myofascial pain so the cause may be in a different place from where you experience it. These are some of the questions to which a specialist physiotherapist should be able to give you some answers after doing an internal examination.

So far my pelvic pt isn't sure. She said that I have over active pelvic floor but can't be certain what the problem is

So have you been offered a further appointment so she can manually work on relaxing the pelvic floor and provide a more precise diagnosis?

or

has she shown you on a model the anatomical connections between the pelvic floor and the buttocks ?

or

shown you exercises how you could learn to relax your pelvic floor? ( Not just breathing!)

Did you feel understood? What did she say about the results of the various MRIs etc?

it’s such a grey area….ive got a few PN symptoms like burning , shooting and electric shock sensation in my buttocks which radiate into my groin , thighs and testicles plus my sit bones hurt when sitting on some hard surfacesTry to find a specialist who will do the proper checks as other nerves can cause similar symptoms to PN…pelvic floor dysfunction , hip impingement and spine issues…..when it comes to pelvic area there are only limited specialists who understand what is going inside and what needs to be done….theres a lot of trail and error with this issue….you will ever give you a straight forward answer of what exercises you should , what medication you should take and how long it takes to heal….good luck
